摘要
目的探讨动态对比增强磁共振成像(DCE-MRI)联合弥散加权成像(DWI)在胃癌患者术前分期诊断中的应用价值。方法选取84例胃癌患者,所有患者均接受DCE-MRI、DWI检查,并以病理结果作为“金标准”,分析DCE-MRI联合DWI在胃癌患者术前分期诊断中的应用价值。结果入选的84例胃癌患者经病理结果证实,Ⅰ期11例,占13.10%;Ⅱ期28例,占33.33%;Ⅲ期31例,占36.90%;Ⅳ期14例,占16.67%。以病理结果为“金标准”,DCE-MRI检查与病理结果具有理想的一致性(Kappa=0.676);DWI检查与病理结果具有理想的一致性(Kappa=0.658);DCE-MRI、DWI联合检查与病理结果具有极好的一致性(Kappa=0.882)。结论DCE-MRI联合DWI在胃癌术前分期诊断中具有较高的应用价值,其诊断结果与病理结果具有极高的一致性,可为临床制定针对性的手术方案提供依据。
Objective To investigate the application value of dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)combined with diffusion weighted imaging(DWI)in diagnosis of preoperative staging of patients with gastric cancer.Methods 84 patients with gastric cancer were selected,all patients received DCE-MRI and DWI examination,pathological results were used as the“gold standard”,the application value of DCE-MRI combined with DWI in diagnosis of preoperative staging of patients with gastric cancer was analyzed.Results Of 84 patients with gastric cancer confirmed by pathological results,11 cases were in stage I,accounting for 13.10%;28 cases were in stage II,accounting for 33.33%;31 cases were in stage III,accounting for 36.90%;14 cases were in stage IV,accounting for 16.67%.Pathological results were regarded as the“gold standard”,the results of DCE-MRI examination were ideally consistent with the pathological results(Kappa=0.676);the results of DWI examination were greatly consistent with the pathological results(Kappa=0.658);the combined examination of DCE-MRI and DWI had excellent consistency with pathological results(Kappa=0.882).Conclusion DCE-MRI combined with DWI has high application value in diagnosis of preoperative staging of gastric cancer,the diagnostic results are highly consistent with the pathological results,which can provide the basis for the formulation of targeted surgical programs in clinic.
