超声造影引导微波消融术与传统手术治疗乳腺良性结节的效果比较

Comparative study on the effects of contrast-enhanced ultrasound-guided microwave ablation and traditional surgery in treatment of benign breast nodules

目的探讨超声造影引导的微波消融术与传统开放手术治疗乳腺良性结节的临床效果。方法选择2018年1月至2019年6月收治的乳腺良性结节患者,根据治疗手段不同分为传统开放手术组(手术组)80例和超声造影引导的微波消融术组(微波消融组)86例。比较2组患者手术时间、术中出血量、切口愈合时间、术后1 d VAS疼痛评分、术后血肿形成率、术后瘢痕形成率、术后6个月病灶残存率和满意率的差异。结果2组手术时间差异无统计学意义(P>0.05)。微波消融组的术中出血量、切口愈合时间、术后1 d VAS疼痛评分、血肿形成率、瘢痕形成率均明显低于手术组(P<0.05)。术后6个月随访,2组的病灶残存率差异无统计学意义(P>0.05),但微波消融组的患者满意率明显高于手术组(P<0.05)。结论与传统开放式手术相比,采用超声造影引导的微波消融术治疗乳腺良性结节能明显降低术中出血量以及术后血肿和瘢痕形成率,缩短切口愈合时间,减轻术后疼痛程度,提高患者满意度,值得推广应用。

Objective To investigate the clinical effects of contrast-enhanced ultrasound-guided microwave ablation and traditional open surgery in treatment of benign breast nodules.Methods A total of 166 patients with benign breast nodules who were treated in our hospital from January 2018 to June 2019 were enrolled in the study,who were divided into traditional open surgery group(control group,n=80)and contrast-enhanced ultrasound-guided microwave ablation group(observation group,n=86)according to the difference of treatment ways.The operation time,intraoperative blood loss,incision healing time,VAS pain score at 1 day after operation,postoperative hematoma formation rate,postoperative scar formation rate,and lesion residual rate and satisfaction rate at 6 months after operation were observed and compared between the two groups.Results There was no significant difference in operation time between the two groups(P>0.05).However the intraoperative blood loss,incision healing time,VAS pain scores at 1 day after operation,postoperative hematoma formation rate,postoperative scar formation rate in observation group were significantly lower than those in control group(P<0.05).After 6-month follow up,there was no significant difference in lesion residual rate between the two groups(P>0.05),but the satisfaction rate in observation group was significantly higher than that in control group(P<0.05).Conclusion Compared with the traditional open surgery,the contrast-enhanced ultrasound-guided microwave ablation in treating benign breast nodules can significantly decrease intraoperative blood loss and formation rate of postoperative hematoma and scar,shorten incision healing time,relieve the postoperative pain degree,improve satisfaction rate of patients,thus,it is worthy of clinical application and promotion.
