老年子宫脱垂患者心理健康状况及影响因素分析

The mental health status of elderly patients with uterine prolapse and its influencing factors

目的调查北京市老年子宫脱垂患者心理健康状况,并分析其影响因素。方法随机抽取2015年5月至2018年6月首都医科大学附属北京妇产医院治疗的152例老年子宫脱垂患者作为调查对象,随访阶段采用横断面调查法,通过调查问卷对北京市老年子宫脱垂患者进行问卷调查,问卷调查结果使用SPSS 21.0统计学软件进行处理。结果152例老年子宫脱垂患者躯体化、强迫症状、人际敏感、抑郁、焦虑、敌对、恐惧、偏执、精神病性9个因子得分及总分均高于全国成人常模(P<0.05);老年子宫脱垂患者症状自评量表(Symptom check list-90,SCL-90)总分与社会支持、主观支持、对支持的利用度、积极应对、心理弹性得分呈正相关(P<0.05),与消极应对得分呈负相关(P<0.05);所有患者年龄60~80岁;意识清楚,能顺畅交流;子宫脱垂程度、相关疾病知识知晓、合并慢性病、子宫脱垂病史、主观支持、对支持利用度、积极应对、消极应对、心理弹性是老年子宫脱垂患者心理健康的影响因素(P<0.05)。结论老年子宫脱垂患者心理健康状况总体较差,临床应有针对性地对患者进行专业引导和心理干预,改善他们的心理健康状况。

Objective To investigate the mental health status of elderly patients with uterine prolapse in Beijing and its influencing factors.Methods A total of 152 elderly patients with uterine prolapse who were treated in our hospital from May 2015 to June 2018 were enrolled in the study.A cross-sectional survey was performed during follow up.And the patients were surveyed by questionnaire,and the results were statistically analyzed.Results The nine factors scores and total scores of somatization,obsessive-compulsive symptoms,interpersonal sensitivity,depression,anxiety,hostility,ear,paranoia and psychosis in the 152 elderly patients were significantly higher than those of health population(P<0.05).The total scores of the symptom check list-90(SCL-90)in the patients were positively correlated with social support,subjective support,utilization of support,active coping,and psychological elasticity score(P<0.05),however,which were negatively correlated with passive coping scores(P<0.05).Moreover the clearly conscious,smooth communicate,uterine prolapse degree,knowledge of related diseases,combined chronic disease,subjective support,support utilization,positive coping,negative coping,and psychological elasticity were the influencing factors of mental health status of the patients(P<0.05).Conclusion The mental health status of elderly patients with uterine prolapse is generally poor,therefor,it is necessary to perform professional guidance and psychological intervention to improve the mental health status of patients.
