利多卡因治疗早产儿惊厥的1例报道并文献复习

摘要 目的探讨早产儿惊厥的诊断及治疗。方法回顾性分析1例早产儿惊厥病例的临床资料,总结持续静脉泵入利多卡因的用药剂量、疗程及疗效,并复习相关文献。结果患儿经苯巴比妥、咪达唑仑抗惊厥治疗效果不佳,加用利多卡因2 mg/kg负荷量,3 mg·kg^(-1)·h^(-1)持续静脉滴注,并根据患儿神经系统症状体征逐步减量,用药总时长36 h,惊厥控制效果良好,监测期间未发生不良反应。结论利多卡因可能是常规抗惊厥治疗无效早产儿惊厥的用药选择之一。 Objective To explore the diagnosis and treatment of convulsions in premature infants.Methods The clinical data of a preterm infant with convulsion was retrospectively analysed.The dosage,course,and efficacy of continuous intravenous infusion of lidocaine for the treatment of convulsion in this case were summarized,and relevant literature was reviewed.Results The anticonvulsant effect of phenobarbital and midazolam in this case was poor.Thus,lidocaine was given at a loading dose of 2 mg/kg,followed by a continuous infusion at 3 mg·kg^(-1)·h^(-1),and the dose was gradually reduced according to the preterm infant′s neurological symptoms and signs.The total duration of medication was 36 h.The anticonvulsant effect was good,and no adverse reaction occurred during the monitoring period.Conclusion Lidocaine may be one of the medication options for the treatment of convulsions in preterm infants,failing conventional therapy.
