失效模式与效应分析在病案首页质控中的应用

Application of failure mode and effect analysis in quality control of medical record homepage

目的探讨失效模式与效应分析在病案首页质控中的应用价值。方法失效模式与效应分析(Failure mode and effect analysis,FMEA)团队于2020年3月—9月期间内实施病案首页质控的失效模式与效应分析管理,将2020年10月出院病案首页质量数据与2020年1月未实施失效模式与效应分析的情况作比较,观察实施前后,风险优先指数值(RPN)、病案首页缺陷率情况。结果实施后,疾病诊断冲突发生率、手术操作编码错误发生率、主要诊断缺陷率、首页合计缺陷率分别为5.02%、1.33%、1.36%、7.70%,较实施前病案缺陷率更低,差异存在统计学意义(P<0.05)。实施后,各危险因素的RPN值均有了明显的降低。结论失效模式与效应分析在病案首页质控中的应用效果显著。

Objective To explore the effect of failure mode and effect analysis(FMEA)on the quality control of the medical records homepage.Methods The FMEA team implemented the FMEA management of the medical record homepage quality control from March to September 2020.The medical record homepages in October 2020 were compared with those in January 2020 in terms of quality control effect to investigate the risk priority number(RPN)and the defect rate of the medical record homepage before and after the implementation.Results After the implementation,the incidence of disease diagnosis conflicts,the incidence of surgical operation coding errors,the rate of major diagnostic defects,and the total number of defects on the medical record homepage were 5.02%,1.33%,1.36%,and 7.70%,respectively,which was significantly lower than before implementation,respectively(P<0.05).After implementation,the RPN value of each risk factor was also significantly reduced.Conclusion The FMEA is effective in the quality control of medical record homepages.
