运动性引导想象训练联合双抗疗法对脑梗死后睡眠障碍患者睡眠质量、心理状态及神经功能的影响

Effect of Exercise-guided Imagination Training Combined with Dual Antiplatelet Therapy on Sleep Quality,Mental State and Neurological Function of Patients with Post-Stroke Sleep Disorders

目的:探究运动性引导想象训练联合双抗疗法对脑梗死后睡眠障碍(PSSD)患者睡眠质量、心理状态及神经功能的影响。方法:选取2018年8月至2020年8月连云港市第一人民医院神经康复科接受治疗的PSSD患者70例作为研究对象,采用随机数表法分为观察组和对照组,每组35例。观察组患者采用运动性引导想象训练联合双抗疗法治疗,对照组患者仅采用双抗疗法治疗。治疗前、治疗后第2周、治疗后第4周,采用匹兹堡睡眠质量指数(PSQI)量表检测其睡眠质量变化;治疗前后,采用焦虑自评量表评分(SAS)、抑郁自评量表评分(SDS)评估其心理状态情况,神经功能缺损评分(NIHSS)检测其神经功能改善情况,并比较2组患者治疗前后的脑源性神经营养因子(BDNF)浓度。结果:治疗后第2周、第4周,观察组患者的PSQI均低于对照组(均P<0.05);治疗后,观察组患者的SAS、SDS、NIHSS评分、BDNF浓度均低于对照组,差异均有统计学意义(P<0.05)。结论:运动性引导想象训练联合双抗疗法可显著提高PSSD患者的神经功能和睡眠质量,降低患者的不良情绪,值得在临床中推广应用。

Objective:To explore the effect of exercise-guided imagination training combined with dual antiplatelet therapy on sleep quality,mental state and neurological function of patients with post-stroke sleep disorders(PSSD).Methods:A total of 70 patients with PSSD treated in the hospital between August 2018 and August 2020 were enrolled.They were divided into observation group and control group by random number table method,with 35 patients in each group.The observation group received exercise-guided imaging training and dual antiplatelet therapy,while the control group received dual antiplatelet therapy only.The Pittsburgh Sleep Quality Index(PSQI)was used to detect the changes in sleep quality before treatment,at 2 and 4 weeks after treatment.The self-rating anxiety scale(SAS)and self-rating depression scale(SDS)were used to evaluate mental state,and the National Institutes of Health Stroke Scale(NIHSS)was used to detect the improvement of neurological function.The brain-derived neurotrophic factor(BDNF)levels in the two groups before and after treatment were compared.Results:At 2 and 4 weeks after treatment,the observation group had lower PSQI scores than the control group(P<0.05).After treatment,the observation group had lower SAS scores,SDS scores,NIHSS scores and BDNF levels than the control group(P<0.05).Conclusion:Exercise-guided imagination training combined with dual antiplatelet therapy can significantly improve the neurological function and sleep quality of patients with PSSD,and reduce their unhealthy emotions.
