利奥西呱联合西地那非治疗先天性心脏病术后肺动脉高压的疗效及安全性分析

Analysis on Efficacy and Safety of Riociguat Combined with Sildenafil in the Treatment of Pulmonary Arterial Hypertension after Congenital Heart Disease Surgery

目的分析利奥西呱联合西地那非治疗先天性心脏病(CHD)术后肺动脉高压(PAH)的疗效与安全性。方法将2019年3月至2020年5月我院收治的68例CHD术后PAH患者随机分为两组各34例。参照组服用西地那非治疗,研究组服用利奥西呱联合西地那非治疗,两组均治疗3个月。比较两组的治疗效果、血流动力学指标[肺动脉平均压(mPAP)、肺血管阻力(PVR)、混合静脉血氧饱和度(SvO_(2))]及不良反应情况。结果研究组治疗总有效率为94.12%,高于参照组的73.53%(P<0.05)。治疗后,研究组的mPAP、PVR低于参照组,SvO2高于参照组(P<0.05)。两组不良反应发生率比较无统计学差异(P>0.05)。结论利奥西呱联合西地那非治疗CHD术后PAH的效果显著,可有效改善患者的血流动力学,且未明显增加不良反应。

Objective To explore the efficacy and safety of riociguat combined with sildenafil in the treatment of pulmonary arterial hypertension(PAH)after congenital heart disease(CHD)surgery.Methods 68 patients with PAH after CHD surgery in our hospital from March 2019 to May 2020 were randomly divided into two groups equally.The reference group was treated with sildenafil,and the study group was treated with riociguat combined with sildenafil.Both groups were treated for 3 months.The treatment effects,hemodynamic indicators[mean pulmonary artery pressure(m PAP),pulmonary vascular resistance(PVR),mixed venous oxygen saturation(Sv O2)]and adverse reactions of the two groups were compared.Results The total effective rate of treatment in the study group was 94.12%,higher than73.53%in the reference group(P<0.05).After treatment,the m PAP and PVR of the study group were lower than those of the reference group,and the Sv O2 was higher than that of the reference group(P<0.05).No statistical difference was found in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Riociguat combined with sildenafil in the treatment of PAH after CHD surgery has significant effect,which can effectively improve patients’hemodynamics,without significantly increasing the adverse reactions.
