妊娠期糖尿病患者子痫前期发生情况及影响因素分析 被引量:2

摘要 目的分析妊娠期糖尿病(GDM)患者子痫前期(PE)发生情况及影响因素,以期为后续干预措施的制定提供依据。方法选择2018年1月至2020年2月我院收治的83例GDM患者,统计其入院至分娩时的PE发生率,并收集患者的基线资料,包括年龄、孕次、体重指数(BMI)、文化程度、空腹血糖(FPG)及同型半胱氨酸(Hcy)水平,分析GDM患者PE发生的相关影响因素。结果83例GDM患者中,PE发生率为13.25%(11/83)。单因素分析结果显示,GDM患者发生PE不受年龄、孕次、文化程度的影响(P>0.05),但可能受BMI、FPG、Hcy水平的影响(P<0.05)。多因素Logistic回归分析显示,高BMI、FPG、Hcy水平均为GDM患者发生PE的影响因素(OR>1,P<0.05)。结论GDM患者存在一定的PE发生风险,BMI、Hcy及FPG水平过高均为GDM患者发生PE的危险因素,临床可据此进行合理干预,以有效降低PE发生率。 Objective To analyze the occurrence and influencing factors of preeclampsia(PE)in patients with gestational diabetes(GDM),in order to provide basis for the formulation of follow-up intervention measures.Methods 83 GDM patients admitted to our hospital from January 2018 to February 2020 were selected,the PE incidence from admission to delivery were recorded,and the baseline data including age,pregnancy times,body mass index(BMI),education level,fasting plasma glucose(FPG)and homocysteine(Hcy)levels were collected,and the related influencing factors of PE in GDM patients were analyzed.Results Among 83 GDM patients,the incidence of PE was 13.25%.Univariate analysis showed that the occurrence of PE in GDM patients was not affected by age,pregnancy times and education level(P>0.05),but may be affected by BMI,FPG and Hcy level(P<0.05).Multivariate Logistic regression analysis showed that high BMI,FPG and Hcy levels were the influencing factors of PE in GDM patients(OR>1,P<0.05).Conclusions There is certain risk of PE in GDM patients.Too-high BMI,Hcy and FPG levels are the risk factors of PE in GDM patients.Clinical intervention can be carried out according to these factors so as to effectively reduce the incidence of PE.
