二代测序协助诊断多杀巴斯德菌脓胸1例

摘要 多杀巴斯德菌脓胸少见,容易被漏诊。1例81岁患者因"咳嗽、咳痰、发热3 d"入院,诊断为"社区获得性肺炎",经抗感染治疗后仍发热,胸X射线摄影检查提示胸腔积液,行胸腔闭式引流,引流出红褐色液体,将胸腔积液行二代测序,检测出多杀巴斯德菌。多杀巴斯德菌对生长条件要求比较严格,是一种难以培养的细菌,而二代测序的应用有助于快速诊断病原体。 Pasteurella multocida empyema is rare and easy to be misdiagnosed.An 81-year-old male patient showed symptoms with cough,sputum,and fever for 3 days.Community-acquired pneumonia was diagnosed firstly.After anti-infection treatment,the patient was still in fever.Chest radiography showed pleural effusion,closed thoracic drainage was performed and the reddish-brown fluid was drained out.The second-generation sequencing was performed on pleural fluid and Pasteurella multocida was detected.Pasteurella multocida has strict requirements for growth conditions and it difficult to cultivate.The application of second-generation sequencing is helpful to diagnose the pathogen rapidly.
