序贯性通气治疗重症急性左心衰竭伴呼吸衰竭患者的疗效

Efficacy of sequential ventilation in the treatment of severe acute left heart failure with respiratory failure patients

目的:分析序贯性通气治疗重症急性左心衰竭伴呼吸衰竭的疗效。方法:2019年12月-2020年12月收治重症急性左心衰竭伴呼吸衰竭患者76例,随机分为两组,各38例。对照组给予常规通气治疗;观察组采用序贯性通气治疗。比较两组治疗效果。结果:观察组有创通气时间及机械通气时间均明显短于对照组,差异有统计学意义(P<0.05);观察组治疗后氧分压及氧合指数均明显高于对照组,二氧化碳分压明显低于对照组,差异均有统计学意义(P<0.05);观察组治疗后第1秒用力呼气容积、用力肺活量、第1秒用力呼气量占用力肺活量比值、呼气峰值流速等肺功能指标水平均明显高于对照组,差异有统计学意义(P<0.05)。结论:序贯性通气治疗重症急性左心衰竭伴呼吸衰竭患者的疗效显著。

Objective:To analyze the efficacy of sequential ventilation in the treatment of severe acute left heart failure with respiratory failure.Methods:From December 2019 to December 2020,76 patients with severe acute left heart failure and respiratory failure were selected and randomly divided into two groups with 38 cases in each group.The control group was treated with conventional ventilation.The observation group was treated with sequential ventilation.The therapeutic effects between the two groups were compared.Results:The duration of invasive ventilation and mechanical ventilation in the observation group were significantly shorter than the control group,the difference was statistically significant(P<0.05).After treatment,the partial pressure of oxygen and oxygenation index in the observation group were significantly higher than the control group,and the partial pressure of carbon dioxide was significantly lower than the control group,the difference was statistically significant(P<0.05).After treatment,the pulmonary function indexes such as forced expiratory volume 1 second,forced vital capacity,ratio of forced expiratory volume to forced vital capacity 1 second and peak expiratory flow rate in the observation group were significantly higher than the control group,the differences were statistically significant(P<0.05).Conclusion:Sequential ventilation is effective in the treatment of severe acute left heart failure with respiratory failure.
