牙周炎与妊娠期糖尿病关联的病例对照研究

A case-control study of the association between periodontitis and gestational diabetes mellitus

目的:对牙周炎和妊娠期糖尿病(GDM)两种疾病间可能存在的关联进行分析并探究影响GDM发病的可能危险因素。方法:该病例对照研究共纳入样本540例,其中病例组(GDM)135例,对照组405例。对纳入受试者进行牙周检查。通过多元回归和多项式回归分析对影响GDM发病的危险因素进行统计并计算危险度和可信区间。结果:GDM人群牙周炎发病率为40.7%,对照组牙周炎发病率为43.7%。牙周炎和GDM发病间不存在关联。多元回归分析结果提示,影响GDM发病的危险因素为:孕妇年龄,慢性高血压和BMI。结论:在妊娠人群中牙周炎发病率较高,其发病同GDM无统计学相关。

Objective:To study the association and risk factors of gestational diabetes mellitus(GDM)with periodontitis.Methods:540 pregnant women were enrolled in this case-control study.The case(GDM)and control groups included 135 and 405 pregnant women respectively.All the samples received periodontal exams including bleeding on probing,probing depth and clinical attachment loss.Multivariate logistic and multinomial regression were performed to find the possible risk factor to GDM in addition with calculating the odds ratio and confidence interval.Results:The prevalence of periodontitis was 40.7%in GDM group and was 43.7%in the control group.No statistical association was found between periodontitis and GDM.The logistic regression analysis results indicated that the risk factors for GDM were maternal age,chronic hypertension and BMI.Conclusion:The prevalence of periodontitis is high in pregnant women,however it had no impact to GDM.
