摘要
目的:评估负压封闭引流技术(VSD)联合重组人表皮生长因子(rhEGF)治疗糖尿病足溃疡(DFU)难愈创面的效果。方法:将56例DFU患者按治疗方式不同分为观察组和对照组。观察组采用VSD联合rhEGF治疗,对照组采用VSD治疗。对比两组创面肉芽组织出现时间和溃疡愈合时间,治疗1、2、4周的溃疡愈合面积百分比,治疗4周创面愈合总有效率,并发症、不良反应及随访情况。结果:观察组创面肉芽组织出现时间和溃疡愈合时间均早于对照组,差异具有统计学意义(均P<0.05);观察组溃疡面积愈合率在不同治疗阶段均优于对照组,差异均具有统计学意义(均P<0.05);观察组创面愈合效果明显优于对照组(P<0.05);两组患者住院治疗期间均无不良反应及并发症发生,各有1例在随访第7月复发,差异无统计学意义(P>0.05)。结论:使用VSD联合rhEGF治疗糖尿病足溃疡难愈创面,能促进肉芽组织生长,缩短溃疡创面愈合时间,且无不良反应,值得临床推广应用。
Objective:To evaluate the treatment effect of vacuum sealing drainage(VSD)combined with recombinant human epidermal growth factor(rhEGF)for the refractory wound of diabetic foot ulcer(DFU).Methods:A total of 56 patients with DFUs who were admitted to the department of orthopedics were randomly divided into the observation group and the control group according to different treatment methods,with 28 cases in each group.The observation group was treated with VSD combined with rhEGF,while the control group was treated with VSD only.The indicators were compared respectively,including occurrence time of granulation tissue,ulcer healing time,percentage of ulcer healing area on 1 week,2 weeks and 4 weeks after treatment,the total effective rate of wound healing on 4 weeks after treatment,incidence of complications or adverse reactions,and follow-up situation.Results:The occurrence time of granulation tissue and ulcer healing time in the observation group were significantly earlier than those of the control group(both P<0.05).The percentage of ulcer healing area in the observation group was better than that in the control group at different treatment stages,with statistically significant differences(all P<0.05).The wound healing effect of the observation group was significantly better than that of the control group(P<0.05).There was no adverse reaction or complication during the treatment in the two groups.One patient in each group relapsed at the 7 th month of follow-up,with no statistically significant difference(P>0.05).Conclusion:VSD combined with rhEGF in treatment of DFU can promote the growth of granulation tissue and shorten the healing time of ulcer wound,without adverse reactions,which is worthy of clinical application.
