应用夜戴型角膜塑形镜后偏中心情况的研究及其对视觉质量的影响

The Study of Off-center Condition after Night Wear Orthokeratology Lens and Its Influence on Visual Quality

目的:分析配戴夜戴型角膜塑形镜后偏中心的情况及其对视觉质量的影响。方法:选取本院在2018年7月-2020年7月收治的50例需要进行夜戴型角膜塑形镜治疗的患者作为研究对象,回顾性分析其临床资料,测量统计其戴镜不同时间点(7、30、90 d)后的角膜塑形镜基弧区(BC,中央光学区)相对于瞳孔中心的偏心量,观察50例患者中央光学区偏心情况,包括平均偏心量、偏心程度、偏心方向,比较其佩戴夜戴型角膜塑形镜前及90 d后的裸眼视力及矫正视力。结果:50例患者配戴角膜塑形镜后不同时间点的中央光学区平均偏心量比较差异均无统计学意义(P>0.05),同时中央光学区偏心程度多以轻中度为主,重度偏心情况较少,佩戴后不同时间点偏心程度比较差异无统计学意义(P>0.05)。50例患者配戴角膜塑形镜后不同时间点的光学区偏心方向多以水平颞侧偏心及垂直下方偏心为主,佩戴后不同时间点偏心方向比较差异无统计学意义(P>0.05)。50例患者佩戴夜戴型角膜塑形镜90 d后的裸眼视力及矫正视力均要明显高于佩戴前,差异有统计学意义(P<0.05)。结论:对于进行夜戴型角膜塑形镜治疗的患者,配戴过程可发生中央光学区偏中心情况,其偏心程度多以轻中度为主,重度偏心比较少见,偏心方向多以水平颞侧偏心及垂直下方偏心为主,同时配戴夜戴型角膜塑形镜可以在一定程度上改善其视觉质量,值得推广。

Objective:To Analyze the off-center condition after wearing night wear orthokeratology and its effect on visual quality.Method:A total of 50 patients who were admitted to our hospital from July 2018 to July 2020 and needed night wear orthokeratology were selected as the research object,their clinical data were retrospectively analyzed,and the eccentricity of the base arc area (BC,central optical zone) of the orthokeratology lens relative to the center of the pupil in the different time points of wearing the glasses (7,30,90 d) were measured.The off-center condition of the central optical zone of 50 patients,including the average eccentricity,eccentric distance,and eccentric direction were observed.Their naked eyesight and corrected eyesight before and 90 days after wearing night wear orthokeratology were compared.Result:There was no significant difference of the comparison of the average eccentricity of the central optical zone in different time points after wearing the orthokeratology lens (P>0.05).At the same time,the eccentricity of the central optical zone was mostly mild to moderate,and severe eccentricity was rare.The eccentric distance at different time points after wearing the orthokeratology lens was not statistically significant (P>0.05).The eccentricity direction of the optical zone at different time points after wearing the orthokeratology lens in 50 patients were mostly horizontal temporal eccentricity and vertical downward eccentricity.There was no significant difference of the comparison of eccentricity direction at different time points after wearing the orthokeratology lens (P>0.05).The naked vision and corrected visual acuity of 50 patients after wearing night wear orthokeratology for 90 days were significantly higher than before wearing the orthokeratology,the difference was statistically significant (P<0.05).Conclusion:For patients undergoing the treatment of wearing night wear orthokeratology,off-center condition of the central optical zone may occur during the wearing process.The off-center condition is mostly mild to moderate,and severe off-center condition is rare.The eccentricity direction is mostly horizontal temporal eccentricity and vertical downward eccentricity,and wearing night wear orthokeratology can improve visual quality to a certain extent,which is worthy of promotion.
