摘要
目的:探讨儿童性早熟诊断中性激素检测的应用价值。方法:将本院2018年6月-2020年4月收治的74例性早熟儿童作为试验组;将本院2018年6月-2020年4月体检的74例健康儿童作为参照组。均进行性激素检测,对比性激素检测结果[孕酮(P)、垂体催乳素(PRL)、雌二醇(E_(2))、睾酮(T)、促卵泡生成素(FSH)及促黄体生成素(LH)],女童卵巢容积、直径>4 mm卵泡数、子宫容积及GnRH激发试验结果。结果:试验组P(0.29±0.11)ng/ml及LH(0.82±0.30)mIU/ml与参照组的(0.32±0.13)ng/ml、(0.79±0.30)mIU/ml比较差异无统计学意义(P>0.05);T(15.13±5.78)ng/dl、PRL(9.41±2.03)ng/ml、FSH(3.72±1.12)mIU/ml及E_(2)(50.59±12.35)pg/ml,均明显高于参照组的(13.03±3.39)ng/dl、(6.52±2.12)ng/ml、(2.33±0.56)mIU/ml、(30.13±2.62)pg/ml(P<0.05)。试验组女童卵巢容积(2.39±0.69)ml、直径>4 mm卵泡数(4.59±2.13)个、子宫容积(4.01±0.79)ml,均大于参照组的(1.03±0.25)ml、(3.59±1.49)个、(1.13±0.35)ml(P<0.05)。试验组LH峰值(23.15±4.69)mIU/ml,高于参照组的(6.79±1.43)mIU/ml(P<0.05);FSH峰值(14.95±3.35)mIU/ml,低于参照组的(18.02±3.46)mIU/ml(P<0.05)。结论:性激素检测方法有效运用,可充分明确儿童性早熟特征,对于疾病治疗方案针对性研究提供有力依据。
Objective:To explore the application value of sex hormone detection in diagnosis of precocious puberty in children.Method:A total of 74 cases of precocious puberty children in our hospital from June 2018 to April 2020 were selected as the experimental group;74 cases of healthy children in our hospital from June 2018 to April 2020 were selected as the reference group.All were tested for sex hormones,and the results of sex hormone detection[progesterone (P),pituitary prolactin (PRL),estradiol (E_(2)),testosterone (T),follicle-stimulating hormone (FSH) and luteinizing hormone (LH)],girls ovarian volume,number of follicles with diameter >4 mm,uterine volume and GnRH challenge test result were compared.Result:There were no significant differences in P (0.29±0.11) ng/ml and LH (0.82±0.30) mIU/ml of experimental group and (0.32±0.13) ng/ml,(0.79±0.30) mIU/ml of reference group (P>0.05).T (15.13±5.78) ng/dl,PRL (9.41±2.03) ng/ml,FSH (3.72±1.12) mIU/ml and E_(2) (50.59±12.35) pg/ml of experimental group were significantly higher than (13.03±3.39) ng/dl,(6.52±2.12) ng/ml,(2.33±0.56) mIU/ml and (30.13±2.62) pg/ml in the reference group (P<0.05).In the experimental group,the ovarian volume (2.39±0.69) ml,the number of follicles with diameter >4 mm (4.59±2.13) and the uterine volume (4.01±0.79) ml of girls were significantly higher than (1.03±0.25) ml,(3.59±1.49) and (1.13±0.35) ml of the reference group (P<0.05).The peak value of LH in the experimental group was (23.15±4.69) mIU/ml,which was significantly higher than (6.79±1.43) mIU/ml in the reference group (P<0.05).The peak value of FSH (14.95±3.35) mIU/ml was significantly lower than (18.02±3.46) mIU/ml of the reference group (P<0.05).Conclusion:The effective application of sex hormone detection method can fully identify the characteristics of precocious puberty in children and provide a strong basis for targeted research on disease treatment programs.
