英夫利昔单抗联合硫唑嘌呤治疗克罗恩病肛瘘手术患者的临床研究

Infliximab Plus Azathioprine in Patients Undergoing Anal Fistula Surgery for Crohn Disease

目的:探究英夫利昔单抗联合硫唑嘌呤治疗克罗恩病肛瘘手术患者的临床疗效。方法:选取在2018年12月-2020年2月于本院接受治疗的62例克罗恩病肛瘘手术患者,依据数字表法随机分为对照组和观察组,各31例。对照组于术后给予硫唑嘌呤片,观察组在对照组的基础上联合使用英夫利昔单抗,两组均以10周为1个周期,共治疗4个周期。比较两组临床疗效、治疗前后免疫功能指标和炎性因子水平。结果:观察组临床总有效率为90.32%,高于对照组的67.74%(P<0.05);治疗后两组外周血CD8^(+)水平及血清IL-10水平较治疗前均升高,且观察组较对照组高(P<0.05);治疗后两组外周血CD4^(+)、CD4^(+)/CD8^(+)水平及血清IL-12和TNF-α水平均降低,观察组低于对照组(P<0.05)。结论:英夫利昔单抗联合硫唑嘌呤可有效调节克罗恩病肛瘘手术患者免疫功能,同时抑制体内炎症发生,且临床效果显著。

Objective:To investigate the clinical efficacy of Infliximab combined with Azathioprine in patients undergoing anal fistula surgery for Crohn disease.Method:A total of 62 surgical patients with Crohn disease anal fistula,who were treated in our hospital between December 2018 and February 2020,were randomly divided into control group and observation group according to the number table method,with 31 cases in each group.The control group was given Azathioprine Tablets postoperatively,and the observation group combined with Infliximab on basis of the control group,with 10 weeks as one cycle in both groups for a total of four cycles.The clinical efficacy,immune function indexes,and inflammatory factor levels before and after treatment were compared between the two groups.Result:The clinical total effective rate in the observation group was 90.32%,which was higher than 67.74% in the control group (P<0.05);after treatment,the peripheral blood CD8^(+) level and serum IL-10 level of the two groups were higher than those before treatment,and the observation group was higher than the control group (P<0.05);after treatment,the levels of CD4^(+),CD4^(+)/CD8^(+) in peripheral blood and the levels of serum IL-12 and TNF-α decreased in both groups,and the observation group was lower than the control group (P<0.05).Conclusion:Infliximab combined with Azathioprine can effectively regulate the immune function of patients undergoing Crohn disease anal fistula surgery,while inhibiting inflammation in the body,and the clinical effect is significant.
