摘要
目的:探讨肥胖儿童血尿酸(UA)水平与糖脂代谢的相关性。方法:选择2018年10月-2020年3月于笔者所在医院就诊的130例肥胖儿童设为观察组,选取同期于笔者所在医院体检中心体检的130例健康儿童(BMI正常)设为对照组。分别检测两组UA、空腹血糖(FPG)、空腹胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、尿素氮(BUN)、血肌酐(Cr),并计算胰岛素抵抗指数(IR)。比较两组检测指标。根据血UA水平将观察组分为高尿酸血症组(HUA组)与正常尿酸血症组(NUA组),比较两组检测指标。结果:观察组血UA及IR均高于对照组,差异有统计学意义(P<0.05)。观察组HUA、IR、高TG血症、低HDL-C血症发生率均高于对照组,差异有统计学意义(P<0.05)。HUA组BMI、IR和Cr均高于NUA组,差异有统计学意义(P<0.05);HUA组HDL-C低于NUA组,差异有统计学意义(P<0.05)。Pearson相关分析显示,肥胖儿童血清UA与BMI、IR、Cr呈正相关,与HDL-C呈负相关;即使BMI正常的儿童,血UA水平也与BMI、Cr呈正相关(P<0.05)。结论:肥胖儿童中HUA、糖脂代谢异常发生率较高,且血UA水平与BMI、IR、HDL-C、Cr相关,应该重视肥胖儿童的糖脂代谢问题。
Objective:To investigate the correlation between serum uric acid(UA)level and glucose and lipid metabolism in obese children.Method:A total of 130 obese children admitted in our hospital from October 2018 to March 2020 were selected as the observation group,and 130 healthy children(normal BMI)who underwent physical examination in the physical examination center of our hospital during the same period were selected as the control group.The UA,fasting plasma glucose(FPG),fasting insulin(FINS),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),urea nitrogen(BUN),serum creatinine(Cr)were detected and insulin resistance index(IR)were calculated.The detection indexes were compared between the two groups.According to the level of blood UA,the observation group was divided into the hyperuricemia group(HUA group)and the normal uricemia group(NUA group),and the detection indexe of the two groups were compared.Result:The UA and IR of the observation group were higher than those of the control group,the differences were statistically significant(P<0.05).The incidence of HUA,IR,high TG and low HDL-C in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).BMI,IR and Cr in the HUA group were higher than those in the NUA group,and the differences were statistically significant(P<0.05).The HDL-C in the HUA group was lower than that in the NUA group,and the difference was statistically significant(P<0.05).Pearson correlation analysis showed that serum UA was positively correlated with BMI,IR and Cr in obese children,and negatively correlated with HDL-C.Even in normal BMI children,serum UA level was positively correlated with BMI and Cr(P<0.05).Conclusion:The incidence of HUA,abnormal glucose and lipid metabolism is higher in obese children,and the blood UA level is correlated with BMI,IR,HDL-C and Cr.Therefore,attention should be paid to glucose and lipid metabolism problems in obese children.
