掌侧入路万向锁定钢板内固定术对桡骨远端骨折患者疗效观察

Effect of Palmar Approach Universal Locking Plate Internal Fixation on Distal Radius Fracture

目的:探讨掌侧入路万向锁定钢板内固定术对桡骨远端骨折患者的临床疗效。方法:回顾性选取2019年6月-2020年3月本院收治的72例桡骨远端骨折患者。根据不同的治疗方案进行分组,35例患者行掌侧入路普通钢板固定术,列入对照组。37例患者行掌侧入路万向锁定钢板固定术,列入观察组。比较两组临床疗效、手术时间、术中出血量、骨折愈合时间、住院时间、关节功能评分、尺偏角及掌倾角。结果:观察组的总有效率高于对照组,差异有统计学意义(P<0.05)。观察组的手术时间、骨折愈合时间及住院时间均短于对照组,观察组术中出血量少于对照组,差异均有统计学意义(P<0.05)。治疗前两组关节功能评分、尺偏角、掌倾角及桡骨高度差异无统计学意义(P>0.05)。治疗后两组关节功能评分、尺偏角、掌倾角及桡骨高度均升高,观察组关节功能评分、尺偏角、掌倾角及桡骨高度明显高于对照组,差异有统计学意义(P<0.05)。结论:与掌侧入路普通钢板固定术相比,掌侧入路万向锁定钢板固定术治疗桡骨远端骨折治疗效果更好,骨折愈合时间短,腕部关节功能恢复更好,具有一定的应用价值。

Objective:To investigate the clinical effect of volar approach universal locking plate internal fixation for distal radius fracture.Method:A total of 72 patients with distal radius fracture admitted to our hospital from June 2019 to March 2020 were retrospectively selected.According to different treatment regimen,35 patients received palmar approach common plate fixation and were included in the control group,and 37 patients received palmar approach universal locking plate fixation and were included in the observation group.Clinical efficacy,operation time,intraoperative blood loss,fracture healing time,hospital stay,joint function score,ulnar angle and palmar angle were compared between the two groups.Result:The total effective rate of observation group was higher than that of control group,the difference was statistically significant(P<0.05).The operation time,fracture healing time and hospital stay of the observation group were shorter than those of the control group,and the intraoperative blood loss of the observation group was less than that of the control group,the differences were statistically significant(P<0.05).Before treatment,there were no significant differences in joint function score,ulnar angle of deviation,palmar angle and radius height between the two groups(P>0.05).After treatment,joint function score,ulnar angle of deviation,palmar angle and radius height were increased in the two groups,and the observation group were significantly higher than those in the control group,with statistical significances(P<0.05).Conclusion:Compared with common plate fixation with palmar approach,universal locking plate fixation with palmar approach has better therapeutic effect on distal radius fracture,shorter healing time and better recovery of wrist joint function.It has certain application value.
