内镜辅助下微血管减压术治疗面肌痉挛临床效果观察

Clinical analysis of endoscope-assisted microvascular decompression in the treatment of hemifacial spasm

目的观察内镜辅助下微血管减压术(MVD)治疗面肌痉挛(HFS)的疗效和安全性。方法选取新乡市第二人民医院2019-03-01-2020-04-30行内镜辅助下MVD的70例HFS患者为研究对象,按照基线信息具有可比性的方法,分为观察组(34例)和对照组(36例)。观察组行内镜辅助下MVD治疗,对照组行显微镜辅助下MVD治疗。分析2种手术辅助方式下MVD治疗的安全性和有效性。结果观察组34例患者的责任血管均被发现,对照组36例患者中有6例未明确,观察组责任血管明确率高于对照组,χ^(2)=6.20,P=0.013。观察组治疗有效率为94.12%,高于对照组的72.22%,χ^(2)=6.20,P=0.013。观察组术后并发症发生率为5.56%,低于对照组的25.00%,χ^(2)=4.83,P=0.028。结论内镜辅助下MVD治疗HFS,术中可清晰显示细微血管结构,从而避免遗漏责任血管,手术较显微镜辅助更安全有效。

Objective To observe the efficacy and safety of endoscopic microvascular decompression in the treatment of hemifacial spasm.Methods Totally 70 patients with hemifacial spasm who underwent microvascular decompression Second People’s Hospital in Xinxiang from March 1,2019 to April 30,2020 were included.According to the method of comparability of baseline information of the two groups,they were divided into observation group(34 cases)and control group(36 cases).Endoscopy assisted patients were included in the observation group,and microscope assisted patients were enrolled in the control group.The safety and effectiveness of the two surgical assistance methods were compared.Results The responsible vessels of 34 patients in the observation group were found,and 6 patients in the control group were not clear.The clear rate of responsible vessels in the observation group was significantly higher than that in the control group(χ^(2)=6.20,P=0.013).The effective rate of the observation group was94.12%,which was significantly higher than 72.22%of the control group(χ^(2)=6.20,P=0.013).During the follow-up period,the incidence of postoperative complications in the observation group was 5.56%,which was significantly lower than 25.00%in the control group(χ^(2)=4.83,P=0.028).Conclusion Endoscope-assisted microvascular decompression is more safe and effective in the treatment of hemifacial spasm.
