壳聚糖护创敷料治疗深Ⅱ度烧伤疗效观察

Clinical Efficacy of Chitosan Wound Dressing in the Treatment of Burns of Deep Second Degree

目的分析探讨壳聚糖护创敷料治疗深Ⅱ度烧伤的临床疗效。方法选取2017年2月至2020年2月航空工业襄阳医院收治的80例深Ⅱ度烧伤患者作为研究对象,并按照治疗方法将其分为观察组与对照组,每组40例。观察组患者局部创面采用壳聚糖护创敷料换药治疗,对照组患者局部创面采用磺胺嘧啶银乳膏换药治疗,对比两组患者创面疼痛程度、二次创伤评分、创面愈合时间及瘢痕增生情况。结果治疗第10天,观察组患者面部表情疼痛量表(FPS-R)评分为(1.9±0.2)分,明显低于对照组患者的FPS-R评分(2.2±0.4)分(t=4.243,P<0.001);换药过程中,观察组患者二次创伤评分为(1.7±0.6)分,明显低于对照组患者的二次创伤评分(2.2±0.8)分(t=3.162,P=0.002);观察组患者创面愈合时间为(21.2±4.0)d,明显短于对照组患者的创面愈合时间(23.8±4.3)d(t=2.800,P=0.006);随访3个月时,观察组患者温哥华瘢痕量表(VSS)评分为(4.7±1.1)分,明显低于对照组患者的VSS评分(6.2±1.5)分(t=5.100,P<0.001)。结论深Ⅱ度烧伤患者采用壳聚糖护创敷料治疗,可明显缩短创面愈合时间,减轻创面疼痛及二次创伤,并可有效改善瘢痕增生情况,值得临床推广应用。

Objective To explore the clinical efficacy of Chitosan Wound Dressing in the treatment of burns of deep second degree.Methods Eighty patients with deep second degree burns,admitted to Xiangyang Hospital of Aviation Industry between February 2017 and February 2020,were enrolled,and divided into observation group(n=40)treated with dressing change of Chitosan Wound Dressing,and control group(n=40)treated with dressing change of Sulfadiazine Silver Cream.The wound pain degree,secondary trauma score,wound healing time and scar hyperplasia were compared between the two groups.Results On day 10 of treatment,the Faces Pain Scale-Revised(FPS-R)score of patients in observation group was(1.9±0.2)points,obviously lower than(2.2±0.4)points of control group(t=4.243,P<0.001).During dressing change,the secondary trauma score of patients in observation group was(1.7±0.6)points,markedly lower than(2.2±0.8)points of control group(t=3.162,P=0.002).The wound healing time of patients in observation group was(21.2±4.0)d,significantly shorter than(23.8±4.3)d of control group(t=2.800,P=0.006).At three-month follow-up,the Vancouver Scar Scale(VSS)score of patients in observation group was(4.7±1.1)points,obviously lower than(6.2±1.5)points of control group(t=5.100,P<0.001).Conclusion In the treatment of burns of deep second degree,application of Chitosan Wound Dressing can significantly shorten the wound healing time,alleviate wound pain and secondary trauma,and effectively lessen scar hyperplasia,deserving to be promoted in clinical practice.
