芭克硅胶软膏联合压力疗法防治面部深Ⅱ度烧伤后瘢痕疗效观察

Observation on the Clinical Efficacy of Silicone Gel(Scar Gel)Combined with the Pressure Therapy in the Prevention and Treatment of Scar Following Facial DeepⅡDegree Burns

目的观察芭克硅胶软膏联合压力疗法防治面部深Ⅱ度烧伤后瘢痕的临床疗效。方法选取2015年10月至2017年12月毕节市第一人民医院收治的38例面部深Ⅱ度烧伤患者作为研究对象,并按照随机数表法将其随机分为研究组(20例)与对照组(18例),研究组患者于创面愈合后予以芭克硅胶软膏联合定制压力面罩治疗,对照组患者于创面愈合后2~3周单纯应用定制压力面罩治疗,对比观察两组患者治疗第1~3、1~6及4~6个月瘢痕评分改善情况。结果研究组患者治疗第1~3、1~6个月的瘢痕改善分值为(4.65±0.49)分、(7.05±0.39)分,明显高于对照组患者治疗第1~3、1~6个月的瘢痕改善分值(2.61±0.92)分、(5.11±1.08)分(t=8.678、7.511,P均<0.001);研究组患者治疗第4~6个月的瘢痕改善分值为(2.40±0.68)分,与对照组治疗第4~6个月的瘢痕改善分值(2.50±0.92)分无明显差异(t=0.377,P=0.709)。结论与单纯压力疗法相比,芭克硅胶软膏联合压力疗法防治面部深Ⅱ度烧伤后瘢痕的效果更好,且以3个月内瘢痕改善效果最为明显,值得临床推广应用。

Objective To observe the clinical efficacy of Silicone Gel(Scar Gel)combined with the pressure therapy in preventing and treating scar following facial deepⅡdegree burns.Methods 38 patients with facial deepⅡdegree burns,admitted to Bijie First People’s Hospital from October 2015 to December 2017,were enrolled and divided into study group(20 cases)and control group(18 cases)using random number table.Patients in study group were given Silicone Gel and the customized pressure mask to treat scar formation after wound healing,whereas patients in control group were only given the customized pressure mask 2-3 weeks after the wound healing.The scar scale scores on month 1-3,1-6 and 4-6 of treatment were respectively compared between the two groups to evaluate the scar improvement.Results The scores of scar scale on month 1-3 and 1-6 of treatment in study group were(4.65±0.49)points and(7.05±0.39)points,being significantly higher than the corresponding(2.61±0.92)points and(5.11±1.08)points in control group(t=8.678 and 7.511,both P<0.001).The scar score on month 4-6 of treatment in study group was(2.40±0.68)points,showing no significant difference as compared with the corresponding(2.50±0.92)points in control group(t=0.377,P=0.709).Conclusion Compared with the simple application of the pressure therapy,the combined application of Silicone Gel and the pressure therapy can realize better therapeutical effect in the prevention and treatment of scar following deepⅡdegree burns on the face,especially within 3 months of treatment,which is worthy of clinical promotion.
