基于治疗药物监测的癫痫患者不规范用药现象分析和病例报道

Analysis and Case Report of Non-standard Drug Use in Epilepsy Patients Based on Therapeutic Drug Monitoring

目的:分析报道癫痫患者不规范用药现象,提高临床医生识别能力,促使癫痫患者选择正确的就医途径。方法:收集2016年1月~2020年9月在本院进行治疗药物监测的21149例癫痫患者数据,对癫痫患者不规范用药现象进行分析和报道。结果:癫痫患者不规范用药情况主要有2种:一种是将常见抗癫痫药物加入自制的中草药制剂中;另一种是将常见抗癫痫药物伪装成“祖传秘方”。对癫痫疾病的认识不足和非法医学广告的虚假宣传,是癫痫患者及其家人上当受骗的主要原因。通过回顾性调查分析发现,个别患者甚至因此类不规范用药而发生不良反应。结论:癫痫患者不规范用药情况较为普遍,治疗药物监测是识别和解决此类问题的重要方法。严厉打击非法添加药物和非法医学广告,加强癫痫相关知识的普及,提高临床医生警惕性,及时纠正不规范用药问题,是有效避免癫痫患者因不正当就医而延误病情甚至发生严重不良后果的有力举措。

Objective:Analyze and report the phenomenon of non-standard drug use in epilepsy patients,to improve the recognition ability of clinicians and promote epilepsy patients to choose the right way of medical treatment.Methods:The data of 21149 patients with epilepsy who were monitored for therapeutic drugs in our hospital from January 2016 to September 2020 were collected,and the phenomenon of non-standard drug use in those patients was analyzed and reported.Results:There are mainly two kinds of non-standard drug use in epilepsy patients:one is to add common antiepileptic drugs to self-made Chinese herbal preparations,and the other is to disguise common antiepileptic drugs as"ancestral secret prescriptions".Insufficient understanding of epilepsy and false publicity of illegal medical advertisements are the main reasons why epilepsy patients and their families are deceived.Through retrospective investigation and analysis,it was found that some patients even had adverse reactions due to this kind of non-standard drug use.Conclusion:Non-standard drug use is common in epilepsy patients.Therapeutic drug monitoring is an important method to identify and solve such problems.Severely crack down on illegal drug additives and illegal medical advertising,strengthen the popularization of epilepsy related knowledge,improve the vigilance of clinicians,and timely correct the problem of non-standard drug use,which are powerful measures to effectively prevent epilepsy patients from delaying their condition or even serious adverse consequences due to improper medical treatment.
