以VHI为指导的嗓音训练对甲状腺癌术后患者嗓音声学值、嗓音质量及嗓音障碍程度的影响

Effect of voice training guided by VHI on acoustic value, quality of voice and degree of voice disorder in patients after thyroid cancer surgery

目的探讨以嗓音障碍指数量表(VHI)为指导的嗓音训练对甲状腺癌术后患者嗓音声学值、嗓音质量及嗓音障碍程度的影响。方法根据组间基线资料匹配原则,选择2017-10—2020-07在郑州大学第一附属医院行甲状腺部分或全切除术治疗的甲状腺癌患者159例分为2组。对照组79例予以常规围术期护理,观察组80例在对照组基础上给予以VHI为指导的嗓音训练。观察2组嗓音声学值、嗓音质量、嗓音障碍程度。结果观察组患者的振幅微扰(shimmer)水平高于对照组,最大声时长于对照组,基频微扰(jitter)水平低于对照组,差异均有统计学意义(P<0.05)。观察组患者的总嘶哑度、嗓音障碍指数量表简化中文版(VHI-10)评分,以及VHI量表各维度评分和总分等指标,均低于对照组,差异均有统计学意义(P<0.05)。结论以VHI为指导的嗓音训练,可通过改善甲状腺癌术后患者的嗓音声学值水平,提高嗓音质量,改善嗓音障碍程度。

Objective To investigate the effect of voice training guided by the Voice Disorder Index(VHI)on the acoustic value,voice quality,and degree of voice disturbance in patients after thyroid cancer surgery.Methods According to the principle of matching baseline data between groups,159 patients with thyroid cancer underwent partial or total resection of thyroid cancer in the First Affiliated Hospital of Zhengzhou University were selected from 2017-10 to 2020-07.The control group 79 cases received routine perioperative care.Group 80 cases were given voice training guided by VHI on the basis of the control group.Observe 2 groups of voice acoustic value,voice quality,and degree of voice disorder.Results The amplitude shimmer level of the observation group was higher than that of the control group,the maximum sound duration was longer than that of the control group,and the fundamental frequency perturbation(jitter)level was lower than that of the control group.The differences were statistically significant(P<0.05).The total hoarseness of the observation group,the simplified Chinese version of the Voice Disorder Index(VHI-10)score,and the scores and total scores of the various dimensions of the Voice Disorder Index(VHI)scale were all lower than those of the control group.All were statistically significant(P<0.05).Conclusion VHI-guided voice training can improve the acoustic level of the voice of patients after thyroid cancer surgery,improve the quality of voice,and improve the degree of voice disorder.
