丹芪祛瘀止痛颗粒治疗气虚血瘀型慢性萎缩性胃炎

Effect of Danqi Quyu Zhitong Granules in the treatment of patients with chronic atrophic gastritis of the blood stasis due to qi deficiency pattern

目的分析丹芪祛瘀止痛颗粒对气虚血瘀型慢性萎缩性胃炎患者中医证候及炎症反应的影响。方法选择气虚血瘀型慢性萎缩性胃炎患者50例为研究对象,采用随机数字表法分为对照组与观察组,各25例。对照组采用维酶素片、泮托拉唑片、克拉霉素分散片、甲硝唑片进行治疗,观察组在对照组的基础上采用丹芪祛瘀止痛颗粒进行治疗,2组均治疗3个月。比较2组治疗前和治疗3个月后中医证候、胃黏膜组织病理评分、血清炎性因子水平及胃黏膜尾侧型同源转录因子1(CDX 1)、尾侧型同源转录因子2(CDX 2)阳性表达情况。结果与治疗前比较,治疗3个月后,2组中医证候主症、次证评分、总分、肠上皮化生、炎症活动度、黏膜炎症、异型增生、腺体萎缩评分及血清肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、肿瘤坏死因子受体I(TNFRI)水平均降低,观察组低于对照组(P<0.05)。与治疗前比较,治疗3个月后,2组胃黏膜CDX 1、CDX 2阳性率降低,观察组低于对照组(P<0.05)。结论丹芪祛瘀止痛颗粒治疗气虚血瘀型慢性萎缩性胃炎患者,可有效改善患者中医证候,促进患者病情转归,同时降低机体炎症反应,抑制胃黏膜CDX 1、CDX 2阳性表达。

Objective To analyze the effect of the Danqi Quyu Zhitong Granules on the TCM syndromes and inflammatory response in patients with chronic atrophic gastritis of the blood stasis due to qi deficiency pattern.Methods A total of fifty patients with chronic atrophic gastritis of the blood stasis due to qi deficiency pattern were selected as the research subjects,and they were randomly divided into a control group and an observation group,with 25 cases in each group.The control group was treated with verapamil tablets,pantoprazole tablets,clarithromycin dispersible tablets and metronidazole tablets,while the observation group was treated with the Danqi Quyu Zhitong Granules on the basis of the control group.Both groups were treated for 3 months.The TCM syndromes,pathological score of gastric mucosa,serum inflammatory factor levels and positive expression of the caudal homologous transcription factor 1(CDX 1)and caudal homologous transcription factor 2(CDX 2)in gastric mucosa were compared between the two groups before treatment and after 3 months of treatment.Results Compared with before treatment,the scores of the main and secondary TCM syndromes,total scores,intestinal metaplasia,inflammatory activity,mucosal inflammation,dysplasia,gland atrophy scores,and the levels of the serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and tumor necrosis factor receptor I(TNFRI)of the two groups were all decreased,and the observation group was significantly lower than the control group(P<0.05).Compared with before treatment,the positive rates of CDX 1 and CDX 2 in gastric mucosa of the two groups were decreased 3 months after treatment,and the observation group was significantly lower than the control group(P<0.05).Conclusion The Danqi Quyu Zhitong Granules can effectively improve the TCM syndromes of patients with chronic atrophic gastritis of the blood stasis due to qi deficiency pattern,promote the prognosis,reduce the body’s inflammatory response,and inhibit the positive expression of CDX 1 and CDX 2 in gastric mucosa.
