十全大补汤联合肠内营养支持对胃癌术后患者营养情况和免疫功能的影响

Effect of Shiquan Dabu Decoctions combined with the enteral nutrition support on the nutritional status and immune function of postoperative patients with gastric cancer

目的分析十全大补汤联合肠内营养支持对胃癌术后患者营养情况和免疫功能的影响。方法选取200例胃癌术后患者,采用随机数字表法分为研究组与对照组,各100例。2组均行胃癌根治手术进行治疗,对照组予肠内营养支持,研究组在对照组基础上予十全大补汤治疗,2组均治疗至出院。观察2组肠胃功能恢复时间,比较2组治疗前后营养、体液免疫功能及细胞免疫功能指标。结果研究组术后首次排气、排便及肠鸣音恢复时间均短于对照组(P<0.05)。治疗后,2组血清血红蛋白(Hb)、转铁蛋白(TRF)、总蛋白(TP)、免疫球蛋白A(IgA)、IgG、IgM及外周血CD4^(+)、CD3^(+)、CD4^(+)/CD8^(+)水平均高于治疗前,研究组高于对照组(P<0.05);2组外周血CD8^(+)水平均低于治疗前,研究组低于对照组(P<0.05)。结论十全大补汤联合肠内营养支持可有效改善胃癌术后患者营养情况及免疫功能,有利于缩短患者肠胃功能恢复时间。

Objective To analyze the effect of the Shiquan Dabu Decoctions combined with the enteral nutrition support on the nutritional status and immune function of postoperative patients with gastric cancer.Methods A total of 200 postoperative patients with gastric cancer were enrolled as the study subjects and randomly divided into a study group and a control group,with 100 cases in each group.After the radical surgery for gastric cancer,the control group was given an enteral nutrition support,and the study group was treated with the Shiquan Dabu Decoctions on the basis of the control group.Both groups were treated until their discharge from the hospital.The recovery time of gastrointestinal function in the two groups was observed,and the indexes of nutrition,humoral immune function and cellular immune function were compared before and after treatment.Results The time of thefirst exhaust,defecation and recovery of bowel sounds in the study group was shorter than that in the control group(P<0.05).After treatment,the levels of the serum hemoglobin(Hb),transferrin(TRF),total protein(TP),immunoglobulin A(IgA),IgG,IgM and peripheral blood CD4^(+),CD3^(+),CD4^(+)/CD8^(+)in the study group were higher than those in the control group(P<0.05),while the level of the peripheral blood CD8^(+)in the two groups was lower than that before treatment,and the study group was lower than the control group(P<0.05).Conclusion The Shiquan Dabu Decoctions combined with the enteral nutrition support can effectively improve the nutritional status and immune function of the postoperative patients with gastric cancer,and is beneficial to shorten the recovery time of gastrointestinal function.
