连续肾脏替代治疗(CRRT)患者上机前访视需求的调研

A study of the pre machine treatment visit needs in patients undergoing continuous renal replacement therapy(CRRT)

目的调研并了解连续肾脏替代治疗(CRRT)患者上机前访视需求,及对其存在的影响因素进行分析。方法采用自制的问卷形式对本市某三级甲等医院共205例CRRT患者上机前进行访视需求,问卷回收后进行统计分析。结果患者的文化程度、居住的所在环境、共同居住的家人心理状态和患者自身医保类型,是CRRT患者上机前访视需求进行分析其存在的影响因素;CRRT患者上机前访视需求中,对于上机前以及上机治疗中、后续的相关费用维度考虑因素得分最高,其他因素中以环境准备、心理支持维度最低。结论CRRT患者在上机前访视需求进行分析有较高要求,其中关于总体费用、操作过程的了解以及疼痛程度了解需求较为强烈。因此建议相关科室进行CRRT时,应该于患者上机前根据患者个体化因素给予全面的、多元化的、充分的上机前心理护理,知识讲解,从而降低患者对于治理产生的恐惧感、应激反应,提高患者的就医、就护感。

Objective To investigate and get to know the pre treatment visit needs of patients with the continuous renal replacement therapy(CRRT),and to analyze the relevant influencing factors.Methods A total of 205 patients with the CRRT in a class A hospital in our city were surveyed on their pre treatment visit needs by a self-made questionnaire,and the questionnaires were collected and analyzed statistically.Results The patients’education level,living environment,the psychological state of family members and the type of medical insurance were the influencing factors for their pre treatment visit needs.In these needs,the cost factors considered before,during and after the machine treatment scored the highest,while the environmental preparation and psychological support scored the lowest among the other factors.Conclusion There is a high requirement for the analysis of the CRRT patients’pre machine treatment visit needs,among which the needs to understand the overall cost,operation process and pain degree are much stronger.Therefore,it is suggested that relevant departments should give a comprehensive,diversified and sufficient psychological nursing care and knowledge explanation according to the patients’individualized factors before the CRRT,so as to reduce their fear and stress response,and improve their sense of seeking medical treatment and nursing care.
