高频超声结合超声引导下穿刺活检对非哺乳期乳腺炎的诊断价值

Diagnostic value of high frequency ultrasound combined with ultrasound-guided puncture biopsy in non-lactating mastitis

目的探讨高频超声结合超声引导下穿刺活检对非哺乳期乳腺炎的诊断价值。方法选取2016年7月~2018年6月期间于我院就诊的经病理证实为非哺乳期乳腺炎患者106例,均接受超声检查,所有患者接受超声引导下穿刺活检,回顾分析106例患者的超声声像图特点。结果106例患者穿刺前超声检查的检出率为74.5%(79/106)。穿刺病理结果包括肉芽肿性小叶性乳腺炎34例,乳腺导管扩张症6例,慢性化脓性乳腺炎21例,淋巴细胞性乳腺炎1例,慢性乳腺炎44例。结论非哺乳期乳腺炎的超声表现复杂、多变,各种类型的乳腺炎超声表现具有相似性、重叠性,部分容易与乳腺恶性肿瘤相混淆。高频超声结合超声引导下穿刺活检对非哺乳期乳腺炎的诊断及鉴别诊断有重要的临床应用价值。

Objective To explore the diagnostic value of high frequency ultrasound combined with ultrasound-guided puncture biopsy for non-lactating mastitis.Methods We collected 106 cases of non-lactating mastitis confirmed by pathology in our hospital from July 2016 to June 2018.All patients underwent ultrasound-guided puncture biopsy.We retrospectively analyzed the ultrasonographic characteristics of the 106 cases.Results The detection rate of ultrasound before puncture was 74.5%(79/106)in 106 cases.The pathological results of puncture included 34 cases of granulomatous lobular mastitis,6 cases of mammary duct ectasiaa,21 cases of chronic suppurative mastitis,1 case of lymphocytic mastitis and 44 cases of chronic mastitis.Conclusion Ultrasound manifestations of non-lactating mastitis are complex and changeable.Ultrasound manifestations of various types of mastitis are similar and overlapping.It is difficult to distinguish them.Some of them are easily confused with malignant breast tumors.High-frequency ultrasound combined with ultrasound-guided puncture biopsy has important clinical value in the diagnosis and differential diagnosis of non-breast-feeding mastitis,and provides accurate diagnostic basis for follow-up treatment in time.
