超声应变成像评价规范化治疗2型糖尿病患者周围动脉的影响 被引量:1

摘要 目的探讨超声应变成像评价规范化治疗2型糖尿病患者周围动脉的影响。方法65例2型糖尿病(T3pe 2 diabetes,T2DM)患者根据颈动脉内膜中层厚度(IMT)分为IMT增厚组30例(1.0 mm≤IMT<1.5 mm)和IMT正常组35例(IMT<1.0 mm)。应用Xstrain自动分析软件分别获取右侧颈总动脉、肱动脉、股动脉和胫前动脉各节段最大圆周应变平均值(Smax)和应变率的平均值(SRmax)。所有患者经规范化治疗6个月后重复以上超声检查,比较两组患者治疗前后以上各动脉Smax、SRmax的变化率。应用SPSS 22.0统计软件包进行统计学分析。结果规范化治疗前和治疗后,IMT增厚组的右侧颈动脉、肱动脉、股动脉和胫前动脉的Smax和SRmax低于IMT正常组,差异有统计学意义(P<0.05)。规范化治疗前后比较,IMT正常组的右侧颈动脉、肱动脉、股动脉和胫前动脉的Smax和SRmax高于治疗前,差异有统计学意义(P<0.05);IMT增厚组的肱动脉和胫前动脉的Smax和SRmax均高于治疗前,差异有统计学意义(P<0.05)。IMT增厚组的胫前动脉Smax变化率低于治疗前,差异有统计学意义(P<0.05)。结论超声应变成像可用来评价规范化治疗对2型糖尿病患者周围动脉,尤其是中小动脉应变的影响。 Objective To evaluate the effect of ultrasound strain imaging on the standardized treatment of peripheral arteries in patients with type 2 diabetes mellitus.Methods 65 newly diagnosed patients with type 2 diabetes mellitus(T2DM)in the second affiliated hospital of wenzhou medical university were selected as the subjects.According to carotid intima media thickness(IMT),30 patients were in the IMT thickened group(1.0 mm≤IMT<1.5 mm)and 35 patients were in the IMT normal group(IMT<1.0 mm).The maximum peripheral strain mean(Smax)and strain rate mean(SRmax)of right common carotid artery,brachial artery,femoral and tibial artery were obtained by using the Xstrain automatic analysis software.After standardized treatment for 6 months,ultrasound examination was repeated and two groups of patients’the above artery’s change rate of Smax,SRmax were compared.SPSS 22.0 statistical software package was used for statistical analysis.Results Before standardized and after standardized treatment,Smax and SRmax of carotid,brachial,femoral and anterior tibial arteries in the thickened IMT group were lower than those in the normal IMT group.After standardized treatment,the Smax and SRmax of carotid,brachial,femoral and anterior tibial arteries in the normal IMT group were higher than before treatment,the differences were statistically(P<0.05).The Smax and SRmax of brachial artery and anterior tibial artery in the IMT thickened group were higher than before treatment,and the differences were statistically significant(P<0.05).The Smax change rate of the anterior tibial artery in the IMT thickened group was lower than before treatment,with statistically significant differences(P<0.05).Conclusion Ultrasonic strain imaging can be used to evaluate the effect of standardized treatment on peripheral artery strain in patients with type 2 diabetes mellitus.
