一种新型下颌前移矫治器治疗阻塞性睡眠呼吸暂停低通气综合征的初步研究 被引量:1

Preliminary Study of the Efficacy of a Novel Mandibular Advancement Device in the Therapy of Obstruction Sleep Apnea-Hypopnea Syndrome

摘要 目的以全数字化流程设计制作一种新型下颌前移矫治器(MAD),评价其对成人阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的初步临床治疗效果。方法应用新型下颌前移矫治器治疗31例轻中度OSAHS患者,在治疗前以及12周治疗后分别填写Epworth嗜睡量表和鼾声评分表,并采用多导睡眠仪进行监测,使用SPSS 19.0统计软件对测量结果进行分析。结果经临床应用及比较观察,患者戴用新型MAD后临床症状(睡眠鼾声、呼吸暂停、日间酣睡)得到有效改善,AHI明显下降(P<0.05),SaO 2明显上升(P<0.05)。结论新型MAD可以明显减轻OSAHS患者的临床症状,和传统的制作方式相比,该技术省去了传统的印模制取和手工制作的步骤,值得进一步深入应用研究。 Objective The aim of this study is to present a fully digital workflow for fabrication of a novel nonadjustable MAD by CAD/CAM,and to evaluate the efficacy of the MAD in the treatment of obstruction sleep apnea-hypopnea syndrome(OSAHS).Methods Thirty-one cases of mild to moderate OSAHS were treated by novel MAD.Before and after treatment,patients were asked to do the polysomnography.At the same time,the subjective symptoms were collected by questionnaires.Results There were significant improvement of symptoms of OSAHS(snoring,daytime sleepiness and witnessed apnea)while wearing the device in comparison to before wearing the device for the treatment of the patients with moderate OSAHS,and of which their AHI declined obviously(P<0.05),and SaO 2 increased obviously(P<0.05).Conclusion The novel MAD can significantly reduce the clinical symptoms of patients with OSAHS.Compared with the traditional fabrication method,this workflow eliminates the impression taking and manual manufacturing steps,which is worthy of further in-depth application research.
