宫颈冷刀锥切术与环形电切术治疗高级别宫颈上皮内瘤变的临床效果比较

Comparison of Clinical Effect of Cervical Cold Knife Conization and Loop Electrosurgical Excision Procedure in the Treatment of High-Grade Cervical Intraepithelial Neoplasia

目的比较宫颈冷刀锥切术(CKC)与环形电切术(LEEP)治疗高级别宫颈上皮内瘤变的临床效果。方法选取2017年1月至2019年1月河南大学淮河医院收治的140例高级别宫颈上皮内瘤变患者,采用随机数表法将其分为对照组与观察组,各70例。对照组接受CKC,观察组接受LEEP。对比两组术后临床疗效、病灶组织面积和术后病灶残留率、手术相关情况、术后宫颈恢复及复发情况。结果术后1个月,观察组治疗总有效率高于对照组(P<0.05)。术后,观察组病灶组织面积较对照组小(P<0.05)。观察组术后病灶残留率较对照组低(P<0.05)。观察组手术时间、术中出血量、切口愈合时间、住院时间均短于对照组(P<0.05)。术后1 a,两组宫颈长度均短于术前(P<0.05)。术后1 a,两组复发率比较,差异无统计学意义(P>0.05)。结论CKC和LEEP疗效相当,但LEEP具有术中出血量少及手术时间短等优点。

Objective To compare the clinical effect of cervical cold knife conization(CKC)and loop electrosurgical excision procedure(LEEP)in the treatment of high-grade cervical intraepithelial neoplasia.Methods A total of 140 patients with high-grade cervical intraepithelial neoplasia admitted to Huaihe Hospital of Henan University from January 2017 to January 2019 were divided into control group and observation group according to the random number table method,with 70 cases in each group.Control group received CKC,and observation group received LEEP.The postoperative clinical efficacy,lesion tissue area,postoperative lesion residual rate,operation-related conditions,postoperative cervical recovery and recurrence were compared between the two groups.Results One month after operation,the total effective rate of observation group was higher than that of control group(P<0.05).The lesion tissue area of observation group was smaller than that of control group(P<0.05).The postoperative lesion residual rate of observation group was lower than that of control group(P<0.05).The operation time,intraoperative blood loss,incision healing time and hospitalization time of observation group were all lower than those of control group(P<0.05).One year after operation,the cervical lengths of the two groups were shorter than those before operation(P<0.05).One year after operation,there was no statistical difference in the recurrence rate between the two groups(P>0.05).Conclusion CKC and LEEP have the same curative effect,but LEEP has the advantages of less intraoperative blood loss and short operation time.
