PCOS患者血清瘦素与性激素、胰岛素抵抗相关性研究

Correlation between serum leptin and sex hormones,IR in patients with PCOS

目的测定多囊卵巢综合征(PCOS)患者血清瘦素的水平,分析与性激素、胰岛素抵抗(IR)的相关性。方法选择2017年4月至2020年4月诊治的105例PCOS患者为研究组,基于体重指数(BMI)分成肥胖组(51例)和无肥胖组(54例);并以65例因男方因素不孕的正常育龄女性为参照组。测定3组血清瘦素、性激素及IR。同时测定PCOS患者治疗前后上述指标的变化。结果研究组血清瘦素、黄体生成素(LH)、睾酮(T)、空腹血糖(FPG)及空腹胰岛素(FINS)均显著高于参照组(P<0.05),且PCOS肥胖组、无肥胖组各指标水平差异均有统计学意义(P<0.05),而FSH无明显差异(P>0.05),而卵泡刺激素(FSH)无明显差异(P>0.05)。治疗后,PCOS肥胖组、无肥胖组的血清瘦素、LH、T、FPG及FINS均比治疗前显著(P<0.05),FSH无明显差异(P>0.05)。相关性分析表明PCOS患者血清瘦素水平和T、IR呈正相关(P<0.05)。结论PCOS患者血清瘦素、性激素、血糖显著增高,且肥胖患者更高,治疗后均可显著改善,PCOS患者血清瘦素和T、IR呈正相关性,可作为临床诊治重要指标。

Objective To detect the serum levels of leptin in patients with polycystic ovary syndrome(PCOS),and to explore its correlation with sex hormones and insulin resistance(IR).Methods A total of 105 patients with PCOS who were diagnosed and treated in our hospital from April 2017 to April 2020 were enrolled as research group,who were redivided into obesity group(n=51)and non-obesity group(n=54)according to the body mass index(BMI).At the same time,the 65 childbearing age women with infertility were enrolled as control group.The serum levels of leptin,sex hormones,and the changes of the above indexes before and after treatment were observed and compared among the three groups.Results The serum levels of leptin,luteinizing hormone(LH),testosterone(T),fasting blood glucose(FPG)and fasting insulin(FINS)in research group were significantly higher than those in control group(P<0.05),moreover there were significant differences in above indexes between obesity group and non-obesity group(P<0.05),however there were no significant differences in the levels follicle stimulating hormone(FSH)(P>0.05).After treatment,the serum levels of leptin,LH,T,FPG and FINS in obesity group and non-obesity group were significantly decreased(P<0.05),however there were no significant differences in FSH levels before and after treatment(P>0.05).Correlation analysis showed that serum leptin levels in patients with PCOS were positively correlated with the levels of T and IR(P<0.05).Conclusion The serum levels of leptin,sex hormones,and blood glucose in patients with PCOS are significantly increased,especially in obese patients,and which can be significantly improved after treatment.And the serum leptin levels are positively correlated with those of T and IR,which can be used as an important index in clinical diagnosis and treatment.
