腹腔镜联合腹主动脉旁淋巴结清扫术治疗子宫内膜癌效果、并发症率及对生活质量影响分析

The Effect and Complication Rate of Laparoscopic and Paraaortic Lymphadenectomy in the Treatment of Endometrial Carcinoma and Its Influence on Quality of Life

目的:分析子宫内膜癌腹腔镜联合腹主动脉旁淋巴结清扫术治疗效果、并发症率及对生活质量的影响。方法:回顾性选取2018年6月~2020年6月某院子宫内膜癌患者100例,依据手术方法分为腹腔镜联合腹主动脉旁淋巴结清扫术组(腹腔镜组)和传统开腹腹主动脉旁淋巴结清扫术组(开腹组)各50例,统计分析两组患者的围术期相关指标、术后并发症发生情况、生活质量。结果:腹腔镜组患者的腹主动脉旁淋巴结清扫出血量显著少于开腹组(P<0.05),腹主动脉旁淋巴结清扫数显著多于开腹组(P<0.05),腹主动脉旁淋巴结清扫时间显著短于开腹组(P<0.05),术后肛门排气时间、住院时间显著短于开腹组(P<0.05);腹腔镜组患者的术后并发症发生率8.00%(4/50)显著低于开腹组22.00%(11/50)(P<0.05);腹腔镜组患者手术后较手术前的躯体、认知、情绪、角色、社会、整体功能评分升高幅度均显著高于开腹组(P<0.05)。结论:和传统开腹腹主动脉旁淋巴结清扫术相比,子宫内膜癌腹腔镜联合腹主动脉旁淋巴结清扫术治疗效果较好,更能有效降低患者的并发症发生率,提升患者的生活质量。

Objective:To analyze the effect and complication rate of laparoscopy and paraaortic lymphadenectomy in the treatment of endometrial carcinoma and its influence on quality of life.Methods:A total of 100 cases of patients with endometrial cancer in a hospital from June 2018 to June 2020 were retrospectively selected and divided into two groups according to the surgical methods:laparoscopy combined with paraaortic lymphadenectomy group(laparoscopic group,n=50)and traditional laparotomy para aortic lymph node dissection group(open group,n=50).The perioperative related indicators,postoperative complications,and quality of life of the two groups were statistically analyzed.Results:The amount of bleeding of paraaortic lymph node dissection of the laparoscopic group was significantly less than the that of open group(P<0.05),the number of paraaortic lymph node dissection was significantly more than that of the open group(P<0.05),the time of paraaortic lymph node dissection,postoperative anal exhaust time,and the hospitalization time were significantly shorter than those of the open group(P<0.05).The incidence of postoperative complications of the laparoscopic group was 8.00%(4/50),which was significantly lower than that of the open group(22.00%,11/50)(P<0.05).The scores of body,cognition,emotion,role,society and overall function of the laparoscopic group after surgery were significantly higher than those of the open group(P<0.05).Conclusion:The effect of laparoscopy combined with paraaortic lymph node dissection is better than traditional open abdominal paraaortic lymph node dissection in the treatment of endometrial cancer,which can more effectively reduce the incidence of complications and improve the quality of life of patients.
