急性心肌梗死PCI术后无复流现象的影响因素 被引量:3

Influencing factors of no reflow after PCI in patients with acute myocardial infarction

摘要 目的探讨急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后无复流现象发生状况及其影响因素。方法回顾性分析2018年7月至2020年6月于遂平县中医院行PCI的AMI患者临床资料,从中筛选符合条件的62例患者作为研究对象,根据术后有无复流现象分为无复流组(n=11)和复流组(n=51),分析AMI患者PCI术后无复流现象的发生状况及其影响因素。结果62例AMI患者PCI术后有11例无复流,占比17.74%,有51例复流,占比82.26%;经单因素分析与多项Logistic回归分析结果显示,缺血时间较长、合并糖尿病、围术期未使用替罗非班为AMI患者PCI术后无复流的影响因素(OR>1,p<0.05)。结论缺血时间较长、合并糖尿病、围术期未使用替罗非班为AMI患者PCI术后无复流的影响因素,临床可据此积极采取防治措施,以降低患者术后无复流风险。 Objective To investigate the occurrence and influencing factors of no reflow phenomenon in patients with acute myocardial infarction(AMI)after percutaneous coronary intervention(PCI).Methods The clinical data of AMI patients who underwent PCI in Suiping Hospital of traditional Chinese medicine from July 2018 to June2020 were retrospectively analyzed.62 eligible patients were selected as the research objects.According to whether there was reflow phenomenon after PCI,they were divided into no reflow group(n=11)and reflow group(n=51).The occurrence and influencing factors of no reflow phenomenon after PCI in AMI patients were analyzed.Results There were 11 cases of no reflow(17.74%)and 51 cases of no reflow(82.26%)in 62 patients with AMI after PCI.The results of univariate analysis and multiple logistic regression analysis showed that longer ischemia time,diabetes mellitus and no use of Tirofiban in perioperative period were the influencing factors of no reflow in patients with AMI after PCI(or>1,p<0.05).Conclusions Long time of ischemia,diabetes mellitus and no use of Tirofiban in perioperative period are the influencing factors of no reflow after PCI in patients with AMI.According to this,preventive measures can be taken to reduce the risk of no reflow.
