晚期胃癌患者预感性悲伤现状及其影响因素

The Status Quo of Anticipatory Grief among Patients with Advanced Gastric Cancer and its Influencing Factors

目的了解预感性悲伤(anticipatory grief,AG)在晚期胃癌患者中的现状及特征,分析其影响因素。方法2020年7-11月便利抽样选取某市4所三级甲等医院住院的晚期胃癌患者338例,采用一般资料表、晚期癌症患者预感性悲伤量表(preparatory grief in advanced cancer patients,PGAC)、医院焦虑抑郁评定量表(hospital anxiety and depression scale,HADS)、阿森斯失眠量表对其进行调查。结果PGAC总得分为(33.75±16.020),各维度得分存在差异。回归结果显示,失眠、焦虑及抑郁程度、躯体症状与PGAC得分呈正相关,年龄与PGAC得分负相关(均P<0.05);癌症复发是影响晚期胃癌患者AG的独立危险因素。结论预感性悲伤的心理反应在晚期胃癌患者中普遍存在,并且与焦虑、抑郁等心理问题以及睡眠状况密切相关。医务人员应加强对晚期胃癌患者预感性悲伤的评估,医院应制定合理的心理筛查诊断机制,并对患者给予有针对性的积极干预。

Objective To understand the status quo and characteristics of anticipatory grief(AG)among patients with advanced gastric cancer,and analyze its influencing factors.Methods Convenient sampling was used to select 338 patients with advanced gastric cancer hospitalized in four tertiary hospitals of one city from July to October,2020.The general information questionnaire,the Preparatory Grief in Advanced Cancer patients(PGAC)scale,the Hospital Anxiety and Depression Scale(HADS),and the Athens Insomnia Scale were adopted for investigation.Results The PGAC total score was(33.75±16.020),and there were differences in the scores of each dimension.Regression analysis showed that insomnia,the degree of anxiety and depression,and physical symptoms were positively correlated with the PGAC score,and the age was negatively correlated with the PGAC score(all P<0.05).Cancer recurrence was an independent risk factor of AG among patients with advanced gastric cancer.Conclusions The psychological reaction of anticipatory grief is common among patients with advanced gastric cancer,and closely related with some mental issues,such as anxiety and depression,and sleep conditions.Therefore,medical staff should emphasize the assessment of anticipatory grief among patients with advanced gastric cancer.Hospitals should develop a reasonable psychological screening and diagnosis system to provide active and targeted intervention for the patients.
