Quality Appraisal of Targeted Temperature Management Guidelines for Patients with Cardiac Arrest

目的系统评价国内外心搏骤停患者目标体温管理相关指南的质量,分析各指南的特点,为临床进行基于证据的目标体温管理提供参考。方法系统检索专业学会网站、临床实践指南网站、数据库的心搏骤停患者目标体温管理的相关指南,检索日期自建库至2019年12月30日。采用AGREE II对纳入的指南进行质量评价。结果共筛选出国外6篇相关循证指南,经AGREE II评价的6个领域标准化评分均值分别为:范围与目的97.2%,参与人员43.4%,严谨性63.7%,清晰性84.4%,应用性30.1%和独立性88.1%。6篇指南总体质量评价为1篇A级,5篇B级。结论心搏骤停患者目标体温管理循证指南的整体质量有待提高,部分指南在应用性领域仍有待完善,经过本土化后可以作为我国临床工作的循证证据。

Objective To systematically evaluate the quality of guidelines on targeted temperature management for cardiac arrest patients at home and abroad,and analyze the characteristics of each guideline,so as to provide reference for clinical evidence-based targeted temperature management.Methods Guidelines on targeted temperature management for cardiac arrest patients were systematically retrieved on the websites of the professional associations,the clinical practice guideline networks and databases.Guidelines published from the date of setting up the database to December 30,2019 were searched.The AGREE II was used to evaluate the quality of the included guidelines.Results Six foreign evidence-based guidelines were included.The average standardized scores of the 6 domains of AGREE II were 97.2%(scope and purpose),43.4%(stakeholder involvement),63.7%(rigor of development),84.4%(clarity of presentations),30.1%(applicability),and 88.1%(independence).Considering the overall quality of the 6 guidelines,1 guideline was evaluated with A class and 5 guidelines with B class.Conclusions The overall quality of evidence-based guidelines on targeted temperature management for cardiac arrest patients needs to be improved.Some guidelines should be improved in their applicability.Localized guidelines can be used as evidence for clinical work in our country.
