乳腺癌化疗性肝损伤的影响因素

Influencing Factors of Chemotherapy-Induced Liver Injury of Breast Cancer

目的分析乳腺癌化疗性肝损伤的相关影响因素。方法回顾性分析2018年1月至2018年12月我院收治的乳腺癌患者577例,按照化疗过程中的肝功能情况分为肝功能异常组和肝功能正常组,采用单因素分析和二元Logistic回归分析探讨乳腺癌化疗性肝损伤的影响因素。结果肝功能正常组和肝功能异常组在新辅助化疗、化疗药物、化疗方案、乙肝表面抗原方面有统计学差异(P<0.05),在年龄、BMI、病理类型、肿瘤大小、淋巴结转移、手术方式、化疗前脂肪肝方面无统计学差异(P>0.05)。乳腺癌化疗性肝损伤的独立影响因素为新辅助化疗、化疗药物和乙肝表面抗原。结论乳腺癌化疗性肝损伤与新辅助化疗、化疗药物、化疗方案、乙肝表面抗原相关,对临床化疗药物的个体化选择及更好地控制化疗期间肝损伤发病率具有一定指导意义。

Objective To explore the related influencing factors of chemotherapy-induced liver injury of breast cancer.Methods 577 patients with breast cancer admitted to our hospital from January 2018 to December 2018 were retrospectively analyzed.According to the liver function during chemotherapy,all cases were divided into abnormal liver function group and normal liver function group.Univariate analysis and binary Logistic regression analysis were used to explore the influencing factors of chemotherapy-induced liver injury of breast cancer.Results There were significant differences in neoadjuvant chemotherapy,chemotherapy drugs,chemotherapy regimen and hepatitis B surface antigen between the normal liver function group and the abnormal liver function group(P<0.05);There was no significant difference in age,BMI,pathological type,tumor size,lymph node metastasis,surgical method and fatty liver before chemotherapy between normal liver function group and abnormal liver function group(P>0.05).The independent influencing factors of chemotherapy-induced liver injury of breast cancer were neoadjuvant chemotherapy,chemotherapy drugs and hepatitis B surface antigen.Conclusions Chemotherapy-induced liver injury of breast cancer is associated with neoadjuvant chemotherapy,chemotherapy drugs,chemotherapy regimen and hepatitis B surface antigen,which has certain guiding significance for individualized selection of clinical chemotherapy drugs and better control of the incidence of liver injury during chemotherapy.
