4种结核分枝杆菌潜伏感染检测方法的比较

摘要 目的通过对4种国产结核潜伏感染检测方法(华大、普瑞康、希格、迪澳)与世界卫生组织推荐的γ-干扰素释放试验方法QuantiFERON-TBGold(QFT)进行比较,筛选适合国内最优的检测技术。方法对纳入的1200名研究对象采集血样,分别以QFT为参照进行结核潜伏感染的检测,对各检测方法的检测结果进行诊断试验评价。结果华大IgG抗体检测试剂盒阳性率0.92%,灵敏度、特异度、阳性预测值、约登指数分别是2.67%、99.67%、72.73%、0.02。普瑞康IgG抗体检测试剂盒阳性率13.50%,灵敏度、特异度、阳性预测值、约登指数分别是14.33%、86.78%、26.54%、0.01。希格T细胞检测试剂盒阳性率12.42%,灵敏度、特异度、阳性预测值、约登指数分别是44.33%、98.22%、89.26%、0.43。迪澳T细胞检测试剂盒阳性率32.42%,灵敏度、特异度、阳性预测值、约登指数分别是90.00%、86.78%、69.41%、0.77。结论迪澳T细胞检测试剂盒具有较高检测阳性率和较好的灵敏度和特异度,可加大检测样本量,进一步验证推广。 Objective To compare four domestic detection methods for latent tuberculosis infection(BGI,Prekang,SIG,and DI􀆳AO)with QuantiFERON-TB Gold(QFT),theγ-interferon release test method recommended by the World Health Organization,and screen the best detection technology suitable for the country.Methods Blood samples were collected from 1200 participants,and QFT was used as a reference to detect latent tuberculosis infection.The results of each detection method were evaluated by diagnostic tests.Results The positive rate of BGI IgG antibody detection kit was 0.92%,and the sensitivity,specificity,positive predictive value,and Youden index were 2.67%,99.67%,72.73%,and 0.02,respectively.The positive rate of Prekang IgG antibody detection kit was 13.50%,and the sensitivity,specificity,positive predictive value,and Youden index were 14.33%,86.78%,26.54%,and 0.01,respectively.The positive rate of Sieg T cell detection kit was 12.42%,and the sensitivity,specificity,positive predictive value,and Youden index were 44.33%,98.22%,89.26%,0.43,respectively.The positive rate of Di'ao T cell detection kit was 32.42%,and the sensitivity,specificity,positive predictive value,and Youden index were 90.00%,86.78%,69.41%,and 0.77,respectively.Conclusion The Di'ao T cell detection kit has a high positive rate and good sensitivity and specificity,which can increase the sample size for further verification.
