半夏白术天麻汤加减联合针灸治疗风痰上扰证后循环缺血性眩晕

Effect of Banxia Atractylodes Gastrodia elata Decoction Combined with Acupuncture and Moxibustion on Patients with Circulatory Ischemic Vertigo After Wind-phlegm Disturbance Syndrome

目的分析半夏白术天麻汤加减联合针灸治疗风痰上扰证后循环缺血性眩晕患者效果。方法选取社旗县人民医院2018年11月至2020年11月风痰上扰证后循环缺血性眩晕患者89例,采用随机数字表法分为两组,西医组44例采用常规西药治疗,联合组45例在常规西药治疗基础上给予半夏白术天麻汤加减联合针灸治疗,比较两组治疗效果、治疗前后中医证候积分、椎基底动脉平均血流速度(Vm)、血浆黏度及纤维蛋白原水平。结果联合组总有效率95.56%(43/45)较西医组79.55%(35/44)高(P<0.05);治疗后两组头晕目眩、耳聋耳鸣、视物旋转等积分低于治疗前,且联合组低于西医组(P<0.05);治疗后两组右椎动脉、左椎动脉、基底动脉Vm水平均较治疗前高,且联合组较西医组高(P<0.05);治疗后两组血浆黏度及纤维蛋白原水平低于治疗前,且联合组低于西医组(P<0.05)。结论半夏白术天麻汤加减具有燥湿化痰、平肝健脾之效,联合针灸清热燥湿、平抑肝阳,用于治疗风痰上扰证后循环缺血性眩晕患者可增强治疗效果,缓解相关症状,提高椎基底动脉血流速度,改善血液流变学。

Objective To analyze the effect of Banxia Atractylodes Gastrodia elata decoction combined with acupuncture and moxibustion on patients with circulatory ischemic vertigo after wind-phlegm syndrome.Methods 89 patients with circulating ischemic vertigo from November 2018 to November 2020 in Sheqi County People’s Hospital,Using the random number table method,Western medicine group of 44 patients treated with conventional western medicine,Combined group of 45 cases on the basis of routine western medicine treatment,Compared the therapeutic effect,TCM syndrome score,mean blood flow velocity(Vm),plasma viscosity and fibrinogen level before and after treatment.Results The total effective rate of the combined group was 95.56%(43/45)higher than that of the western medicine group 79.55%(35/44)(P<0.05);After treatment,the scores of dizziness,deafness,tinnitus and visual rotation were lower than before treatment,and the combined group was lower than the western medicine group(P<0.05);After treatment,the Vm levels of right vertebral artery,left vertebral artery and basilar artery were higher than those before treatment,and the combined group was higher than the western medicine group(P<0.05);After treatment,plasma viscosity and fibrinogen levels were lower than before treatment,And the combined group was lower than the western medicine group(P<0.05).Conclusion Banxia Atractylodes Gastrodia elata Decoction has the effect of dampening phlegm,calming liver and strengthening spleen,Combined acupuncture and moxibustion to clear heat and dampness,calm liver yang,For the treatment of wind and phlegm disturbance syndrome after circulation ischemic vertigo patients can enhance the therapeutic effect,Relieve the symptoms,Increasing the flow velocity of the vertebrobasilar artery,improve hemorheology.
