超声引导下甲状腺良性结节微波消融的疗效与安全性分析 被引量:2

摘要 目的:研讨超声引导下甲状腺良性结节微波消融的疗效与安全性。方法:选取2017年1月-2020年12月期间我院270例甲状腺良性结节患者为研究对象,病患共342个甲状腺良性结节,均采用超声引导下微波消融治疗,对比患者术前和术后甲状腺功能及自身抗体的血清学试验指标(血清游离三碘甲腺原氨酸FT3、游离甲状腺素FT4、促甲状腺激素TSH、促甲状腺素受体抗体TRAb、甲状腺球蛋白抗体TGAb、甲状腺过氧化物酶抗体TPOAb)和术后1个月、3个月、6个月、12个月甲状腺缩小率,以及术中术后并发症的发生率。结果:治疗后,患者的甲状腺结节缩小率逐渐升高,甲状腺功能(FT3、FT4、TSH)及甲状腺自身抗体(TRAb、TGAb、TPOAb)术后1个月的血清学指标与术前相比无统计学意义,P>0.05;且上述患者均未发生皮肤烫伤、甲状腺功能不全、喉返神经损伤、甲状旁腺功能不全等并发症。结论:对良性甲状腺结节的患者,应用超声引导下微波消融治疗,效果显著,安全性高,值得广泛推广和应用。 Objective:To investigate the efficacy and safety of ultrasound-guided microwave ablation of benign thyroid nodules.Methods:270 patients with benign thyroid nodules in our hospital(from January 2017 to December 2020)were retrospectively analyzed.A total of 342 benign thyroid nodules were treated with ultrasound-guided microwave ablation.Then the serum test indexes of thyroid function and autoantibodies(serum free triiodothyronine FT3,free thyroxine FT4,free thyroxine FT3,free thyroxine FT4,free thyroxine FT4)before and after operation were compared Thyroid stimulating hormone(TSH),thyrotropin receptor antibody(TRAb),thyroglobulin antibody(TGAb),thyroid peroxidase antibody(TPOAb)and thyroid shrinkage rate at 1 month,3 months,6 months and 12 months after operation,as well as the incidence of intraoperative and postoperative complications.Results:After treatment,the reduction rate of thyroid nodules gradually increased,and the serum indexes of thyroid function(FT3,FT4,TSH)and thyroid autoantibodies(TRAb,TGAb,TPOAb)1 month after operation were not statistically significant compared with those before operation(P>0.05);There were no skin scald,thyroid insufficiency,recurrent laryngeal nerve injury,parathyroid insufficiency and other complications.Conclusion:Ultrasound-guided microwave ablation is effective and safe for patients with benign thyroid nodules,It is worth popularizing and applying.
