止带方联合三黄散治疗HR-HPV感染临床研究

Clinical Study on Zhidai Decoction Combined with Sanhuang Powder in the Treatment of High-risk HPV Infection

目的:观察止带方联合三黄散治疗宫颈HR-HPV感染湿热下注型的临床疗效。方法:60例按照随机数字表法分为对照组和观察组各30例,对照组给予重组人干扰素α-2b联合银尔洁洗液治疗,观察组给予止带方内服联合三黄散外敷治疗。结果:总有效率观察组93.33%,对照组70.00%,两组比较差异有统计学意义(P<0.05)。HR-HPV转阴率观察组73.33%、对照组63.33%,两组比较差异有统计学意义(P<0.05)。治疗后阴部瘙痒、小腹疼痛、白带及宫颈病变程度评分观察组均比对照组低(P<0.05)。两组血清IL-6水平较治疗前降低(P<0.01),观察组血清IL-6、CD4+/CD8+水平均低于对照组(P<0.01);E6/E7mRNA基因的表达观察组低于对照组(P<0.05)。两组不良反应发生率比较差异无统计学意义(P>0.05)。结论:止带方联合三黄散治疗HR-HPV感染湿热下注型能够显著提高其HPV转阴率、降低炎症反应,降低E6/E7mRNA基因表达,更好地阻止HR-HPV持续性感染,改善临床症状,提高宫颈局部免疫功能,降低宫颈癌的发生率。

Objective:To observe the clinical effect of Prescription for Stopping Leucorrhoea combined with Sanhuang Powder in the treatment of cervical HR-HPV infection with damp-heat invasion of lower energizer type.Methods:According to the random number table method,60 cases were divided into the control group and the observation group,with 30 cases in each group.The control group were treated with recombinant human interferonα-2b combined with Yinerjie lotion while the observation group were treated with Prescription for Stopping Leucorrhoea combined with Sanhuang Powder for external application.Results:The total effective rate was 93.33%in the observation group and 70.00%in the control group.The difference between the two groups was statistically significant(P<0.05).The negative rate of HR-HPV was 73.33%in the observation group and 63.33%in the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the scores of genital itching,lower abdominal pain,leucorrhea and cervical lesions in the observation group were lower than those in the control group(P<0.05).Serum IL-6 levels in the two groups after treatment were lower than before treatment(P<0.01),serum IL-6,CD4+/CD8+levels in the observation group were lower than those in the control group(P<0.01);E6/E7mRNA gene expression in the observation group was lower than the control group Group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Prescription for Stopping Leucorrhoea combined with Sanhuang Powder in the treatment of HR-HPV infection with damp-heat invasion of lower energizer type can significantly increase the HPV conversion rate,reduce inflammation,reduce E6/E7mRNA gene expression,and better prevent persistent HR-HPV infection and improve Clinical symptoms can improve the local immune function of the cervix and reduce the incidence of cervical cancer.
