缺血性脑血管病诊断中经颅多普勒超声的应用

Application of Transcranial Doppler Ultrasound in Diagnosis of Ischemic Cerebrovascular Disease

目的探讨缺血性脑血管病诊断中经颅多普勒超声(TCD)的应用及临床价值探讨。方法选取2019年6月—2020年6月于本院的缺血性脑血管病患者74例。随机分为对照组和研究组,各37例。对照组应用颈动脉超声诊断,研究组应用经颅多普勒超声诊断。观察对比两组诊断效果的准确性、灵敏性、特异性、阴性预测和阳性预测;诊断血管狭窄的程度和条数;检测阳性患者的血清情况:其中包含胆固醇(TC)、甘油三酯(TG)、载脂蛋白A-I(ApoA-I)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)和载脂蛋白B(ApoB)。结果研究组准确性、灵敏性、阴性预测和阳性预测高于对照组,特异性低于对照组,差异均具有统计学意义(P<0.05);研究组诊断血管闭塞或狭窄总例数高于对照组,且差异有统计学意义(P<0.05);研究组TC、LDL、ApoB和TG低于对照组,HDL以及ApoA-I高于对照组。差异均具有统计学意义(P<0.05)。结论应用经颅多普勒超声诊断缺血性脑血管疾病效果理想。

Objective To explore the application and clinical value of transcranial Doppler ultrasound(TCD)in the diagnosis of ischemic cerebrovascular disease.Methods seventy-four patients with ischemic cerebrovascular disease in our hospital from June 2019 to June 2020 were selected.Randomly divided into control group and study group,37 cases each.Carotid ultrasound was used in the control group and transcranial Doppler ultrasound was used in the study group.The accuracy,sensitivity,specificity,negative prediction and positive prediction of the two groups were observed and compared.To diagnose the degree and number of vascular stenosis;The serum profiles of positive patients included cholesterol(TC),triglyceride(TG),apolipoprotein A-I(APOA-I),highdensity lipoprotein(HDL),low-density lipoprotein(LDL),and Apolipoprotein B(ApoB).Results The accuracy,sensitivity,negative prediction and positive prediction of the study group were higher than those of the control group,and the specificity was lower than that of the control group(P<0.05);the total number of cases of vascular occlusion or stenosis in the study group was higher than that in the control group,and the difference was statistically significant(P<0.05);TC,LDL,apoB and TG in the study group were lower than those in the control group,and HDL and apoA-I were higher than those in the control group.The difference was statistically significant(P<0.05).Conclusion The application of transcranial Doppler ultrasound in the diagnosis of ischemic cerebrovascular diseases is ideal.
