妊娠期肝内ICP合并GDM对母婴结局的影响

Effect of Intrahepatic ICP Combined With GDM on Maternal and Fetal Outcomes During Pregnancy

目的探讨母儿结局受到妊娠期肝内ICP并GDM的影响。方法回顾性选取2017年1月—2020年1月本院妊娠期肝内ICP患者60例,依据合并GDM情况分为合并组(n=30)、未合并组(n=30)两组,统计分析两组母儿结局。结果合并组患者的子痫前期、Ⅲ度羊水污染、胎膜早破、产后出血发生率16.7%(5/30)、20.0%(6/30)、23.3%(7/30)、26.7%(8/30)均明显高于未合并组6.7%(2/30)、3.3%(1/30)、3.3%(1/30)、6.7%(2/30)(P<0.05)。合并组患者的分娩孕周明显小于未合并组(P<0.05),新生儿体质量明显低于未合并组(P<0.05),小于胎龄儿、羊水污染、新生儿窒息、早产、围产儿畸形发生率26.7%(8/30)、30.0%(9/30)、13.3%(4/30)、20.0%(6/30)、40.0%(12/30)均明显高于未合并组13.3%(4/30)、6.7%(2/30)、3.3%(1/30)、3.3%(1/30)、3.3%(1/30)(P<0.05)。结论母儿结局受到妊娠期肝内ICP并GDM的不良影响。

Objective To investigate the effect of intrahepatic ICP and GDM on maternal and fetal outcomes.Methods 60 cases of patients with intrahepatic ICP during pregnancy in our hospital from January 2017 to January 2020 were retrospectively selected and divided into combined group(n=30)and non combined group(n=30)according to the situation of GDM.The maternal and fetal outcomes of the two groups were statistically analyzed.Results The incidences of preeclampsia,third degree amniotic fluid pollution,premature rupture of membranes and postpartum hemorrhage in the combined group were 16.7%(5/30),20.0%(6/30),23.3%(7/30),26.7%(8/30)in the combined group,which were significantly higher than 6.7%(2/30),3.3%(1/30),3.3%(1/30),6.7%(2/30)in the non combined group(P<0.05).The gestational weeks of delivery in the combined group were significantly smaller than the non combined group(P<0.05),the neonatal weight was significantly lower than the non combined group(P<0.05).The incidences of small for gestational age infants,amniotic fluid pollution,neonatal asphyxia,premature delivery and perinatal malformation were 26.7%(8/30),30.0%(9/30),13.3%(4/30),20.0%(6/30),40.0%(12/30)in the combined group,which were significantly higher than the non combined group 13.3%(4/30),6.7%(2/30),3.3%(1/30),3.3%(1/30),3.3%(1/30)(P<0.05).Conclusion Maternal and fetal outcomes are affected by intrahepatic ICP and GDM during pregnancy.
