缬沙坦联合硝苯地平对原发性高血压合并冠心病患者血压及心功能的影响

Effect of valsartan combined with nifedipine on blood pressure and cardiac function in patients with essential hypertension complicated with coronary heart disease

目的观察缬沙坦联合硝苯地平对原发性高血压合并冠心病患者血压及心功能的影响。方法选取2017年7月-2019年6月于广东省江门市新会区人民医院治疗的原发性高血压合并冠心病患者86例,采用随机数字表法分为观察组和对照组,各43例。对照组单独采用硝苯地平治疗,在此基础上,观察组联合缬沙坦治疗。比较2组临床疗效、治疗前后血压和心功能,心功能包括左室射血分数(LVEF)、左心室质量指数(LVMI)及左心室后壁厚度(LVPWT)。结果观察组治疗总有效率为95.35%,高于对照组的81.40%(χ^(2)=4.074,P=0.044)。治疗14 d后,2组收缩压和舒张压均明显降低,且观察组低于对照组(P<0.01)。治疗14 d后,2组LVEF均明显提高,LVMI和LVPWT均明显降低,且观察组升高/降低幅度大于对照组(P<0.01)。结论缬沙坦联合硝苯地平治疗原发性高血压合并冠心病的效果较好,可有效降低并控制患者血压,改善心功能,值得应用。

Objective To observe the effect of valsartan combined with nifedipine on blood pressure and cardiac function in patients with essential hypertension complicated with coronary heart disease.Methods A total of 86 patients with essential hypertension complicated with coronary heart disease treated in Jiangmen Xinhui District People′s Hospital from July2017 to June 2019 were selected and divided into observation group and control group by random number table method,with43 patients in each group.The control group was treated with nifedipine alone,on this basis,the observation group was treated with valsartan.The clinical efficacy,blood pressure and cardiac function before and after treatment were compared between the two groups,the cardiac function included left ventricular ejection fraction(LVEF),left ventricular mass index(LVMI)and left ventricular posterior wall thickness(LVPWT).Results The total effective rate of the observation group was 95.35%,which was higher than 81.40%of the control group(χ^(2)=4.074,P=0.044).After 14 days of treatment,the systolic blood pressure and diastolic blood pressure in two groups were significantly decreased,and the observation group was lower than the control group(P<0.01).After 14 days of treatment,LVEF in two groups were significantly increased,LVMI and LVPWT were significantly decreased,and the increase/decrease in observation group was greater than that in control group(P<0.01).Conclusion The combination of valsartan and nifedipine in the treatment of essential hypertension with coronary heart disease has a good effect,which can effectively reduce and control the blood pressure of patients and improve cardiac function.
