磁共振扩散张量成像诊断轻度创伤性脑损伤的价值

摘要 目的探讨磁共振扩散张量成像(DTI)在轻度创伤性脑损伤(mTBI)中的诊断价值。方法对30例mTBI患者和30例对照纽行DTI扫描和后重建处理,获得各向异性分数图(FA)、表观扩散系数图(ADC)。在两组图像上分别测量两侧额叶白质区、顳叶白质区、内囊后肢、胼胝体滕部、压部的FA值、ADC值,并统计学分析。结果mTBI组和对照组两侧额叶白质区、颜叶白质区、内囊后肢、胼胝体膝.部、压部等8个感兴趣区(ROI)的FA值差异无统计学意义,ADC值除左侧题叶白质外,其余差异均无统计学意义。纤維束伪彩图观察除1例表现为白质纤维束走行紊乱、中断、部分缺损,其余29例显示正常。结论DTI可以较精确地测量mTBI患者的FA值和ADC值,直观显示脑白质纤维束的微观结构变化和完整性,结合常规MRI可为临床提供客观的影像学定量评估。 Objective To investigate the diagnostic value of dfusion tensor imaging(DTI)in mild traumatic brain injury(mTBI),and to provide imaging evidence for early clinical diagnosis and reasonable treatment.Methods Thirty mT BI patients and 30 mached healchy controls underwent DTI scanming and post reconstruction,the Fractional anisotropy(FA)and apparent difusion cofficient(ADC)were obtained.FA and ADC values of the white mater area of frontal lobe,white matter area of temporal lobe,posterior limb of internal capsule,gen of corpus alloumn,and spleniurn of corpus allosum were measured repectively and aalyzed ststscall Results There werc no stistic lly significant dfferences in FA values of 8 regions of interest(ROI)berween the mTBI group and the control group,including the white matter area of the frontal lobe,the white matter area of the temporal lobe。the posterior limb of the intemal capsule,the genu of corpus callosum,and splenium of corpus allosum,the ADC value had no staistical difference except left temporal white matter.All the 29 cases were normal except I case with white matter fiber bunde disorder,iteruption and partial defect in obsevation of false color image of fiber bundle.Conclusion DTI can accurately measure FA and ADC values of paiens with mTBI,and drecly display the microstructure changes and integrity of white matter tracts.Combined with conventional MRI,DTI can provide an objetive imaging quanitative evaluation method for clinical.
