摘要
目的探讨经脑沟间皮层入路显微手术治疗基底节区自发性脑出血患者的效果及其对手术指标、神经功能的影响。方法选取我院收治的103例基底节区自发性脑出血患者作为研究对象,按照随机数字表法分为对照组(51例)和观察组(52例)。对照组给予传统皮层造瘘治疗,观察组接受经脑沟间皮层入路显微手术治疗,观察两组患者的临床疗效、手术指标、神经功能、预后及并发症情况。结果治疗后观察组临床疗效优于对照组(P<0.05);观察组手术时间长于对照组,观察组术后苏醒时间、血肿吸收时间及住院时间短于对照组(P<0.05),观察组术后格拉斯哥预后(GOS)评分、神经功能缺损程度(NIHSS)评分低于对照组,脑水肿体积小于对照组(P<0.05);两组并发症发生率比较,P>0.05。结论经脑沟间皮层入路显微手术治疗基底节区自发性脑出血通过改善患者手术指标,提高患者神经功能缺损评分,缩小脑水肿体积,从而有效提高临床治疗效果,改善预后,且不良反应的发生率较低。
Objective To explore the efficacy of microsurgery via intersulcus cortical approach in the treatment of spontaneous intracerebral hemorrhage in the basal ganglia area and its influence on surgical indicators and nerve function.Methods 103 patients with spontaneous intracerebral hemorrhage in the basal ganglia were selected as the research ob jects and were randomly divided into the control group(n=51)and the observation group(n=52).The control group re ceived traditional cortical fistula treatment,and the observation group received microsurgery via transsulcus cortical ap proach.The clinical efficacy,surgical indicators,neurological function,prognosis and complications of the two groups were observed.Results After treatment,the clinical efficacy of the observation group was better than that of the control group(P<0.05).The operation time of the observation group were longer than that in the control group;the postoperative recov ery time,hematoma absorption time and hospitalization time in the observation group were shorter than those in the control group(P<0.05).The Glasgow Outcome score(GOS)and Neurological Impairment(NIHSS)score in the observation group were lower than those in the control group,and the brain edema volume in the observation group was smaller than thant in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion The microsurgery via intersulcus cortical approach can effectively improve the clinical ef fect and improve the prognosis by improving patient's surgical indicators,increasing patient's neurological deficit score and reducing the volume of brain edema in the treatment of spontaneous intracerebral hemorrhage in the basal ganglia area.It doesn't increase the incidence of adverse reactions.
