随着全球人口老龄化的加速,退变性脊柱疾病的发生率和致残率不断升高,已成为老年社会面临的健康、卫生、经济及社会问题。同时,由于医学认知的提高和医疗技术的进步,越来越多老年退变性脊柱矢状问题得到诊断和治疗。然而,在获得部分临床效果的同时,由于对退变性脊柱矢状位平衡与失衡的科学认识尚不完全清晰,在此基础上实施的手术治疗尚有部分效果不佳,并发症较多,翻修手术率高,存在过多医疗依赖,经济负担重。目前,国际上越来越多的学者强调:务必正确认识老年退变性脊柱矢状位平衡与失衡,严格把握手术指征,慎重选择手术,努力降低各种并发症,提高卫生经济品质,使临床效果和社会效益最大化,最终让患者和社会获益。

With the acceleration of global population aging,the incidence and disability rate of the degenerative spinal diseases are increasing,which has become a health,hygiene,economic and social problem faced by the elderly society.At the same time,more and more degenerative sagittal spinal problems have been diagnosed and treated due to the improvement of medical cognition and medical technology.However,some of the surgical treatments implemented still have poor clinical effects,more perioperative and long-term complications,high revision rate,medical dependence in the later period and heavy economic burden on some bases such as the scientific understanding of sagittal balance and unbalance is not completely clear.At present,more and more scholars from the world emphasize that it is necessary to correctly understand the sagittal balance and unbalance of the senile degenerative spine,strictly grasp the indications for surgery,carefully implement,strive to reduce various complications.The aim is to improve health and economic quality,maximize clinical and social effects,and ultimately benefit the patients and society.
