摘要
目的探讨保留耳大神经束的腮腺区域性切除术对腮腺癌患者面神经功能的影响。方法将79例腮腺癌患者按照随机数字表法分为研究组40例与对照组39例。比较两组手术时间、切口长度、术中出血量及解剖面神经分支数,术后7 d比较两组面神经功能分级,比较两组并发症发生率。结果研究组手术时间及切口长度显著短于对照组(P<0.01),术中出血量及解剖面神经分支数显著少于对照组(P<0.01)。术后7 d研究组面神经功能分级显著优于对照组(P<0.05),并发症发生率显著低于对照组(P<0.05)。结论保留耳大神经束的腮腺区域性切除术治疗腮腺癌患者效果显著,可有效缩短手术时间,减少术中出血量及面神经解剖数量,降低并发症发生率,安全性较高。
Objective To investigate the effect of regional parotidectomy with great auricular nerve bundle preserved on facial nerve function in patients with parotid carcinoma.Methods A total of 79 patients with parotid carcinoma were divided into a study group(40 cases)and a control group(39 cases)according to random number table method.The operation time,incision length,intraoperative blood loss and the number of anatomical facial nerve branches were compared between the two groups.The functional grading of facial nerve and the incidence of complications were compared between the two groups 7 days after operation.Results The operative time and incision length in the study group were significantly shorter than those in the control group(P<0.01).The amount of intraoperative blood loss and the number of anatomical facial nerve branches were significantly lower than those of the control group(P<0.01).The functional grading of facial nerve in the study group was significantly better than that in the control group(P<0.05),and the incidence of complications was significantly lower than that in the control group(P<0.05).Conclusions Regional parotidectomy with great auricular nerve bundle preserved is effective in the treatment of patients with parotid carcinoma,which can effectively shorten the operation time,reduce intraoperative blood loss and the number of anatomical facial nerve branches,and reduce the incidence of complications,with high safety.
