Efficacy of antimicrobial peptide histatin-5 in periodontitis in elderly patients with smoking periodontitis

目的:比较Histatin-5应用于老年吸烟牙周炎患者不同深度牙周袋患牙辅助治疗后的效果。方法:将58名吸烟的慢性牙周炎患者随机分为试验组和对照组,分别在龈下刮治后牙周袋内应用Histatin-5(RISE-AP12R,锐瑟生物)和碘甘油,1次/周,连续4次。各组患牙刮治前及刮治后3个月复诊时使用Florida探针探诊,记录探诊深度(PD)、临床附着水平(CAL)及探诊出血(BOP);根据牙周袋深度对患牙进行分组:浅牙周袋(3<PD≤5mm)、中牙周袋(5<PD≤7mm)及深牙周袋(PD>7mm);取龈沟液检测牙周相关炎症因子(IL-6、IFN-γ、IL-10)表达水平。对比H istatin-5在不同牙周袋深度患牙中的临床治疗效果。结果:两组在治疗3个月后临床指标均较基线有明显改善,治疗后3个月(T3)时试验组与对照组之间PD,CAL,BOP,以及IL-6、IL-10表达水平均存在统计学差异(P<0.05);两组治疗前后CAL变化程度比较变化值存在统计学差异(P<0.05),PD与BOP的变化无明显差异;两组治疗3个月后两组间浅、中牙周袋数量无明显差异,深牙周袋(PD>7mm)数量存在显著差异(P<0.05)。结论:Histatin-5对吸烟牙周炎患者有一定的辅助治疗效果,并且对深牙周袋作用效果更显著。

Objective:To compare the effect of histatin-5 in the adjuvant treatment of periodontal pocket at different depths in elderly patients with smoking periodontitis.Methods:58 smoking patients with chronic periodontitis were randomly divided into experimental group and control group.Histstatin-5 and iodine glycerin were used in the periodontal pocket after subgingival scaling,once a week,for four consecutive times.The patients in each group were examined by Florida probe before and 3 months after dental curettage,and probing depth(PD),clinical attachment level(CAL)and probing hemorrhage(BOP)were recorded.The affected teeth were grouped according to the depth of the periodontal pocket:shallow periodontal pocket(3<PD≤5mm),periodontal pocket(5<PD≤7mm)and deep periodontal pocket antibacterial peptide(PD>7mm).The expression levels of periodontal inflammatory factors(IL-6,IFN-,IL-10)were measured in gingival crevicular fluid.The clinical effects of histatin-5 in different periodontal pocket depths were evaluated.Results:Both groups showed significant improvement in clinical indicators after 3 months of treatment compared with baseline.Three months after treatment(T3),there were statistically significant differences in PD,CAL,BOP,il-6 and IL-10 expression levels between the experimental group and the control group(P<0.05).There were statistically significant differences in the changes of CAL between the two groups before and after treatment(P<0.05),while there was no significant difference between PD and BOP.There was no significant difference in the number of shallow and middle periodontal pockets between the two groups,while there was a significant difference in the number of deep periodontal pockets(PD>7 mm)(P<0.05).Conclusion:Histstatin-5 has a certain adjuvant therapeutic effect on old smokers with periodontitis and has a more significant effect on deep periodontal pockets.
