Association between HLA-DRB1gene polymorphisms and HBV infection

摘要 目的探究人类白细胞抗原(HLA)-DRB1基因多态性与乙型肝炎病毒(HBV)感染发生和发展的关系。方法选取2017年11月-2020年2月无锡市第五人民医院收治的80例乙型肝炎病毒感染患者为HBV感染组,同期75名健康体检者为对照组。所有研究对象均进行HLA-DRB1基因多态性分析、肝功能测定,分析HBV感染、肝功能与HLA-DRB1基因多态性关系。结果HBV感染组患者DRB1*1501基因频率低于对照组,DRB1*0701、DRB1*0301基因频率高于对照组(P<0.05);肝功能指标天冬氨酸氨基转移酶(AST)、总胆红素(TBIL)和丙氨酸氨基转移酶(ALT)水平与DRB1*1501基因频率呈负相关(P<0.05),与DRB1*0701、DRB1*0301基因频率呈正相关(P<0.05);HBV感染慢性化(PI)组患者DRB1*1501基因型频率低于自限恢复(SR)组(P<0.05),DRB1*0301基因型频率高于SR组(P<0.05),HBV-DNA阳性组患者DRB1*0301基因型频率高于HBV-DNA阴性组(P<0.05)。结论HLA-DRB1*0301等位基因可能会增加HBV病毒感染、慢性化风险,而HLA-DRB1*1501等位基因可能会降低病毒感染风险及损伤。 OBJECTIVE To explore the relationship between human leukocyte antigen(HLA)-DRB1gene polymorphism and occurrence,development of hepatitis B virus(HBV)infection.METHODS A total of 80patients with HBV infection who were treated in Wuxi Fifth People′s Hospital from Nov 2017to Feb 2020were assigned as the HBV infection group,meanwhile,75healthy people who received physical examination were chosen as the control group.The HLA-DRB1gene polymorphisms of all the study subjects were analyzed,the liver function indexes were determined,and the relationship between the HBV infection,liver function and the HLA-DRB1was observed.RESULTS The frequency of DRB1*1501gene of the HBV infection group was significatly lower than that of the control group,the frequencies of DRB1*0701and DRB1*0301genes of the HBV infection group were significatly higher than those of the control group(P<0.05).The levels of liver function indexes aspartate aminotransferase(AST),total bilirubin(TBIL)and alanine aminotransferase(ALT)were negatively correlated with the frequency of DRB1*1501gene(P<0.05)and were positively correlated with the frequencies of DRB1*0701and DRB1*0301genes(P<0.05).The frequency of DRB1*1501gene of the PI group was significatly lower than that of the SR group(P<0.05),the frequency of DRB1*0301genotype of the PI group was significatly higher than that of the SR group(P<0.05);the frequency of DRB1*0301genotype of the HBV-DNA-positive group was significatly higher than that of the HBV-DNA-negative group(P<0.05).CONCLUSIONThe HLA-DRB1*0301allele may increase the risk of HBV infection and chronicity,while the HLA-DRB1*1501allele may reduce the risk of viral infection and damage.
