针刺环跳和委中对坐骨神经损伤大鼠L4-L5脊神经节雪旺细胞、NCAM及CNTF表达的影响 被引量:6

摘要 目的:观察针刺环跳和委中对坐骨神经损伤大鼠L4-L5脊神经节中雪旺细胞、神经细胞黏附因子(NCAM)、睫状神经营养因子(CNTF)表达的影响。方法:80只大鼠随机分为假手术组、模型组、环跳组、委中组,每组20只。采用坐骨神经钳夹损伤模型,两治疗组于造模后第8天予电针治疗。检测大鼠坐骨神经功能指数(SFI),HE染色、神经透射电镜观察L4-L5节段脊神经节形态学变化,免疫组织化学法检测L4-L5节段脊神经节中雪旺细胞、NCAM、CNTF表达。结果:与假手术组比较,造模后各组SFI显著下降(P<0.05);HE染色显示神经纤维排列紊乱,雪旺细胞增多、肿胀,轴突、髓鞘崩解;雪旺细胞、NCAM、CNTF蛋白阳性表达减少(P<0.05);经电针治疗后,与模型组和委中组比较,环跳组SFI数值显著升高(P<0.05);与模型组比较,环跳组、委中组神经纤维数量增多,排列紊乱程度减轻,雪旺细胞减少,并且上述病理改善程度环跳组优于委中组;与模型组比较,环跳组和委中组中雪旺细胞、NCAM、CNTF蛋白阳性表达显著增加(P<0.05),且环跳组雪旺细胞、NCAM、CNTF蛋白阳性表达比委中组增加(P<0.05)。结论:电针穴位干预可以促进坐骨神经损伤大鼠运动功能的恢复,其机制可能与增加雪旺细胞、NCAM、CNTF蛋白的分泌有关,并且电针环跳穴位优于委中。 Objective:To investigate the effect of acupuncture at Huantiao(GB 30)and Weizhong(BL 40)on Schwann cells,NCAM and CNTF of L4-L5 spinal ganglia in rats with sciatic nerve injury.Methods:A total of 80 SD rats were randomly divided into sham control,model,Huantiao(GB 30)and Weizhong(BL 40)groups,20 in each group.Sciatic nerve clamp injury model was used,electroacupuncture(EA)intervention was applied to Huantiao(GB 30)or Weizhong(BL 40)on the 8th day after modeling.The sciatic function index(SFI)was calculated to assess the injured hindlimb recovery.Histopathological changes of the sciatic nerve were displayed by HE staining and transmission electron microscope.The expressions of NCAM,CNTF and Schwann cells in the sciatic nerve tissue were detected by immunohistochemistry.Results:Compared with the sham control group,the SFI level was significantly decreased in the model group(P<0.05),Huantiao(GB 30)and Weizhong(BL 40)groups after the EA intervention(P<0.05).HE staining showed that nerve fibers disordered,Schwann cells increased and swelled,axons and myelin sheath disintegrated,the positive expression of Schwann cells,NCAM and CNTF decreased(P<0.05).After EA treatment,compared with the model group and Weizhong(BL 40)group,the SFI value in the Huantiao(GB 30)group increased significantly(P<0.05),compared with the model group,the number of nerve fibers in Weizhong(BL 40)group and Huantiao(GB 30)group increased,the degree of arrangement disorder decreased and Schwann cells decreased,and the above pathological improvement degree of the Huantiao(GB 30)group was better than that of the Weizhong(BL 40)group.Compared with the model group,the positive expression of Schwann cells,NCAM and CNTF protein in Weizhong(BL 40)group and Huantiao(GB 30)group were significantly increased(P<0.05),and the positive expression of Schwann cells,NCAM and CNTF protein in the Huantiao(GB 30)group were higher than those in the Weizhong(BL 40)group(P<0.05).Conclusion:EA can promote the recovery of hindlimb motor function in SNI rats,the mechanism may be related to increasing the secretion of Schwann cells,NCAM and CNTF proteins.Huantiao(GB 30)is better than Weizhong(BL 40)in the therapeutic effect.
