miR-4485-3p和miR-1469在胃癌患者血清中的表达及临床意义

Clinical significance of serum miR-4485-3p and miR-1469 expression in gastric cancer patients

目的探讨miR-4485-3p和miR-1469在胃癌患者血清中的表达水平,分析其表达与胃癌临床特征的相关性,评估其单项及与CA19-9和CEA联合检测对胃癌临床诊断的应用价值。方法以实时定量聚合酶链反应法检测胃癌组及对照组血清miR-4485-3p和miR-1469的相对表达量;分析二者血清表达水平的相关性及其与胃癌临床因素的相关性;以受试者工作特征(ROC)曲线分析临床诊断效能;通过计算ROC曲线下面积(AUC)、敏感度、特异度、阳性预测值(PPV)及阴性预测值(NPV),评估单项及联合检测的临床应用价值。结果胃癌组和对照组血清miR-4485-3p表达量分别为0.94±1.34,1.66±2.43,差异有统计学意义(P<0.01),而miR-1469的表达量2组间比较差异无统计学意义(P>0.05)。胃癌组和健康对照组中miR-4485-3p与miR-1469的表达水平不相关(r=0.021,P>0.05)。胃癌血清中miR-4485-3p的表达水平与患者年龄、性别、饮酒以及肿瘤TNM分期等均不相关。miR-4485-3p独立诊断胃癌的ROC曲线下面积为0.702,敏感度和特异度分别为88.6%和26.5%,PPV和NPV分别为55.4%和69.2%。miR-4485-3p与CEA、CA19-9联合检测可将敏感度和NPV分别提高至91.4%和80.0%。结论miR-4485-3p在胃癌血清中低表达,可能成为胃癌临床诊断的潜在血清学标志物。

Objective To investigate the expression levels of miR-4485-3 p and miR-1469 in serum of patients with gastric cancer,analyze the correlation between their expression and clinical characteristics of gastric cancer,and evaluate the application value of single detection and combined detection with CA19-9 and CEA in clinical diagnosis of gastric cancer.Methods The relative expressions of miR-4485-3 p and miR-1469 in serum of gastric cancer group and control group were detected by real-time quantitative polymerase chain reaction;the correlation between the serum expression levels and the clinical factors of gastric cancer was analyzed;the receiver operating characteristic(ROC)curve was used to analyze the clinical diagnostic efficacy;the area under ROC curve(AUC),sensitivity,specificity,positive predictive value(PPV)and negative predictive value(NPV)were calculated to evaluate the clinical application value of single and combined detection.Results The expression levels of miR-4485-3 p in gastric cancer group and healthy group were 0.94±1.34 and 1.66±2.43,the difference was statistically significant(P<0.01),but there was no significant difference in the expression of miR-1469 between the two groups(P>0.05).There was no correlation between the expression of miR-4485-3 p and miR-1469 in gastric cancer group and healthy control group(r=0.021,P>0.05).The expression level of miR-4485-3 p in serum of gastric cancer patients was not correlated with age,gender,drinking and TNM stage.The area under the ROC curve of miR-4485-3 p was 0.702,the sensitivity and specificity were 88.6%and 26.5%,PPV and NPV were 55.4%and69.2%,respectively.The sensitivity and NPV of miR-4485-3 p combined with CEA and CA19-9 increased to91.4%and 80.0%,respectively.Conclusion miR-4485-3 p is low expressed in serum of gastric cancer,which may be a potential serological marker for clinical diagnosis of gastric cancer.
